POSITION SUMMARY
Plan, organize, develop, administer, direct, and supervises Physical Therapy services in accordance with current applicable federal, state and local standards, regulations, and guidelines to ensure the highest degree of quality care is rendered at all times. Effective supervisory skills are needed, along with good communication skills for departmental and interdepartmental relationships. The individual in this position must have initiative, be innovative, and possess a sense of responsibility, integrity and sound judgement. The incumbent is expected to work in accordance with the mission, purpose, and values of the Colusa Indian Community Council.

A rural health clinic, we treat a diverse patient population. From pediatric to geriatric ages, we see primarily orthopedic presentations as well as women’s/pelvic health, adolescent sports injuries, balance/fall prevention, neurologic presentations, and health and wellness. We are lucky to enjoy longer treatment times than many OP clinics, able to dedicate the one-on-one attention that we need for even the more complex cases. Associated with the adjacent Indian Health Clinic, we work closely with the team of primary care and specialty providers to offer exceptional care to individuals in the area with all insurance providers (including MediCal and MediCare).

We began an Aquatic Physical Therapy program last year, which we will be expanding this spring and summer. A Physical Therapist on the team will have opportunities for involvement in program expansion, of the Aquatic Program as well as overall clinic growth.
Colusa is a quaint farming town, located in the northern Sacramento Valley, CA. We are a 2-hour drive to the SF Bay Area, 1 hour to Chico or Sacramento, 2.5 hours to Lake Tahoe.

ESSENTIAL DUTIES and RESPONSIBILITIES
- Must complete patient evaluations to ensure quality standards and address assessment areas that require attention. Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients.
- Provides therapy interventions using appropriate treatment protocols and techniques. Establishes individualized care and ensures plans are communicated to patient and appropriate parties.
- Completes comprehensive and accurate clinical documentation within established deadlines.
- Initiate Physical Therapy Treatment and instruct personnel and/or family/caregivers in certain phases of physical therapy with which they may work with a patient as well as instructing them as to the goals of the physical therapy treatment plan.
- Prepares and submits clinical progress summaries.
- Provide Physical Therapy consultation to families and caregivers, as appropriate;
- Supervises assigned assistants.
- May perform other duties commensurate with the functions and level of the position.
ADDITIONAL DESIRED QUALIFICATIONS
- Bi-lingual in Spanish preferred.
- Must be familiar with Microsoft Office and Electronic Health Records.
MINIMUM QUALIFICATIONS
· Education: Doctor of Physical Therapy from an accredited institution.
· Experience:
§ Physical Therapist I: One (1) to Three (3) years of experience
§ Physical Therapist II: Four (4) to Seven (7) year of experience
§ Physical Therapist III: Eight (8) + years of experience
Licensing: Must possess and maintain License to practice as a Physical Therapist within the state of California.
§ Basic Life Support (BLS) Certification.
SUPERVISORIAL DUTIES:
Responsible for the direct over-site of Physical Therapy Aide’s or Physical Therapy Assistant’s interaction with the patient.

PHYSICAL REQUIREMENTS

§ Requires frequent sitting, standing, walking, talking, and hearing in a standard office setting. Specific vision abilities required by this job include close vision to read printed materials and a computer screen with ability to adjust focus;
§ May occasionally lift and/or reposition patients of varying sizes.
§ Must be able to swim (aquatic program).
WORK ENVIRONMENT

- Work is performed in a typical clinical setting which is well-lit, ventilated, and temperature controlled. The noise level is usually low to moderate.
- The incumbent routinely uses standard office equipment such as computers, telephones, photocopiers, and fax machines.
- The incumbent participates in an aquatic program which takes place in a heated pool.
