Physical Therapist

JOB DESCRIPTION:

A Full time Physical Therapist who is willing to contribute to our elite top ranking, 162-bed Skilled Nursing and Rehabilitation Facility in NY state. A Physical Therapist will develop and provide modalities and therapeutic techniques in order to enhance the health and quality of life of our long term residents and sub acute patients. RESPONSIBILITIES:

• Initiate/complete comprehensive evaluations and treatment plans for patients.
• Supervise plan of care carried out by Physical Therapy Assistants.
• Complete weekly skilled documentation and daily billing in electronic medical records.
• Provide customized therapeutic exercise programs, gait/mobility training, functional ADL training, stair training and neuromuscular/balance reeducation.
• Perform family training and discharge planning, including equipment recommendations.
• Monitor vitals and collaborate with interdisciplinary team in our unique, state of the art telemetry unit.

REQUIREMENTS:

• New York State Physical Therapist licensure
• CPR certification
• Physical strength to lift between 50-100lbs
• Able to collaborate with a wide range of health care disciplines, as well as offer insight to patient and family members
• Possess thorough knowledge of the educational and therapeutic values of occupational therapy, as well as the procedures and principles of administration, organization, and management
• Strong written and verbal communication skills
• Respect for residents’ privacy and rights; display of genuine concern and empathy for the elderly
• Able to perform tasks while upholding high standards of quality, cleanliness, and kindness
