Perinatal and Pediatric Coordinator (PPC) for our Family Medicine Residency Program in Richmond, CA. The PPC's responsibilities include welcoming our patients, making basic psychosocial and nutritional assessments, offering appropriate information, education and referrals, and providing emotional support to a culturally and socioeconomically diverse population. The PPC will work closely with the nurse midwives, primary care providers and community health workers. The ideal candidate is a leader with vision and initiative to help lead and integrate the perinatal department in providing the highest services, continuity of care and support to our patients, as well as one who likes to work with a team, is familiar with local community resources, and has experience working with women, children and families in a community health setting.
This is a full time, benefit eligible position.
Bilingual English/Spanish a must.

COVID-19 Vaccine Policy
In accordance with LifeLong Medical Care's commitment to provide and maintain a workplace that is free of known hazards, we have adopted a Mandatory COVID-19 Vaccine Policy to safeguard the health of our employees and their families; our patients and visitors; and the community at large from infectious diseases, that vaccinations may reduce. This policy will comply with all applicable laws and is based on guidance from the Centers for Disease Control and Order of the California State Public Health Officer. Unless a reasonable medical or religious accommodation is approved, all employees must receive COVID-19 vaccinations.
Responsibilities:
Clients will receive care that addresses psychosocial, health education, and nutritional needs. Care will be in compliance with the guidelines of the Comprehensive Perinatal Services Program (CPSP).
Key Responsibilities: Clinical
- Provide orientation and complete assessments on pregnant clients and/or new parents.
- Provide orientation and complete assessments for pediatric clients.
- Create plan and provide appropriate interventions following Steps to Take guidelines.
- Provide health education, support, referrals, and follow-up to clients on caseload.
- Identify high risk clients and refer to psychosocial services.
- Meet all mandated reporting requirements and report to Licensed Provider within 24 hours.
- Chart prep and documentation.
- Taking/recording blood pressures and other vitals within scope, coordinating with the Licensed Provider as needed.
- Integrating with the Licensed Provider to coordinate DM care for GDM patients.
- Participate in centering groups for both perinatal and pediatric patients.
- Other duties as assigned.
Key Responsibilities: Administrative
- Provide leadership and coordination of perinatal and pediatrics services, MA’s, CHW’s and providers who provide these services.
- Participate in meetings and case conferences.
- Communicate and solve problems using appropriate channels.
- Consult with medical provider and/or psychosocial services to ensure appropriate services.
- Keep supervisor current on caseload.
- Document all interactions with clients within 24 hours.
- Complete all paperwork required for CPSP.
- Keep accurate client tracking data.

May participate in community outreach and marketing acitivities to promote the organization.
- Work on projects related to the perinatal and pediatrics department as assigned.
- Other duties as assigned.
Minimum Qualifications:
- High school diploma or GED.
- One year work experience with women, infants, children, families and adolescents
- Knowledge of or ability to learn community resources
- Ability to interact effectively with community agencies and to advocate for clients needing services
- Comfort with discussing sensitive topics such as sexuality, interpersonal violence, financial issues, pregnancy options and substance abuse
- Ability to use computer and electronic health record software
- Strong sense of responsibility and leadership, change management, ability to take initiative and work independently and as part of a team
- Ability to facilitate groups
Desirable Qualifications:
- Experience working with a diverse population
- Experience with non-profit clinic or hospital setting
- Experience with prenatal, birth and pediatrics
- Experience with lactation support
- MA or doula experience, and/or previous perinatal coordinator experience
- Previous leadership and/or management experience
