In partnership with Department of Health & Mental Health (DOHMH) along with Funds for Public Health NYC (FPHNYC), Phipps Neighborhoods has engaged in an initiative to accept the challenge of working towards the elimination of health disparities within the catchment area of the Claremont section of the Bronx using:

The initiative, by employing Community Health Workers, aim to put in place a model for reducing COVID-19 disparities by reducing risk for COVID-19 infection, building social cohesion, and meeting basic social needs and racial inequities - via strong relationships with institutions while serving as a trusted messenger.

Position Summary

Phipps Neighborhoods is seeking an exceptionally qualified candidate for the role of Outreach Specialist who is enthusiastically excited about joining a team committed to incubating an innovative public health initiative leading to improved health for Bronx residents in the Claremont area.

Salary Range: $42,000 to $45,000 - salary commensurate with education, training and experience

Duties and Responsibilities

Working under the leadership of the Program Director or designee, the Outreach Specialist will:

  • Conduct outreach within a prescribed area of the Bronx to educate / inform residents of health services as it relates to program goals
  • Place outreach calls to potential clients, existing clients, health care providers / facilities to validate current health data;
  • Deliver information and respond to questions in a positive manner to facilitate strong relationships;
  • Assist in the building out and weekly social media posting;
  • Works closely with staff to ensure consistent messaging regarding program goals and resources;
  • Managing follow-up activities in a timely manner:
  • Respond to inbound calls, returned emails with a sense of urgency;
  • Serve as a role model and maintain standards of professionalism
  • Build and facilitate relationships with external community partners and other stakeholders
  • Attend coaching sessions and / or educational workshops to enhance or improve performance
  • Attend and represent Phipps Neighborhoods as well as program mission at all community events in target area
  • Attend staff meeting
  • Continuously build community partnerships through either in-person or virtual meetings;

Qualifications:

  • High School Diploma or HSE required; Associate degree preferred
  • Experience in community engagement, health care or other forms of direct service outreach;
  • Outgoing with strong communication, interpersonal and relationship-building skills
  • Ability to multi-task;
  • Data entry experience;
  • Experience in social media applications
  • Excellent human relation skills
  • Bi-lingual (Spanish speaking)
  • Display excellent telephone etiquette;
  • Demonstrated cultural competence the Claremont community of the Bronx;
  • Work closely with staff and health care personnel to obtain information and provide assistance to residents / clients
  • Perform other tasks as directed
