What You Will Do
As an Outreach APP, you are responsible for patient care in clinic and home-based settings, and for maintaining a positive working relationship between the IHTC medical providers and staff and the patients in the community. Care should be in accordance to the Indiana’s State Nursing and Advanced Practice Code or Physician Scope of Practice.
This APP will provide care and education to patients in our Outreach Clinic settings, as well as in patient homes, located in the NE Indiana & North Central region. Frequent travel to these destinations will be required. Examples of coverage areas include, but is not limited to; Fort Wayne, Shipshewana, Berne, South Bend, Mishawaka, etc. The desired candidate will reside in NE Indiana.

The Opportunity

  • Triage and management of patient issues, including, but not limited to; health history, medication review, physical examination, ordering and interpretation of laboratory and radiology studies, development and implementation of a treatment plan, education/counseling, ongoing monitoring and follow-up when needed.
  • Home visits to include home safety assessment, home infusion training, bleeding disorder education, monitoring of treatment products for home storage.
  • Serves as a liaison to assure coordination of patient care between IHTC, local providers and the community.
  • Collaboration with IHTC multidisciplinary staff to manage patient care issues, resolve problems and make referrals to appropriate resources.
  • Works with the local providers to provide education, planning and follow-up regarding patient issues.
  • Facilitates diagnostic testing and on-going laboratory evaluation of patients and appropriate family members.
  • Works as a liaison between State and Federal agencies and the community/families, providing technical assistance during the application and/or re-evaluation process.
  • Documentation in electronic medical record per policies of IHTC.
  • Authorized to administer Basic Life Support in accordance with current CPR standards.
  • Participates in planning and implementation of IHTC research projects, outreach clinics and community events.

Requirements

  • Minimum Masters degree
  • Minimum 3 years of related experience in advanced practitioner role required. At least 1 year of hematology and/or home care experience preferred.
  • Valid NP or PA licensure in the state of Indiana.
  • Valid driver's license and automobile insurance is required.
