Compensation: DOE
Job Summary: Full time, Monday- Friday, 8:00 – 5:00 pm, potentially an occasional weekend shift with offset through the week
Department Description: The Case Management Manager will work closely with the CNO, Nurse Managers, Providers, and staff within the hospital. Currently the department is mostly focused on the inpatient side of the hospital. The plan is to expand the case management department. The goal would be to connect the inpatient services with our services from our 12 clinics and our Santiam Integration Team, which helps provide resources to our patients within our community. The candidate should be someone who enjoys working in a smaller facility and likes to use his or her creative side. We have a great team to work with. They are hardworking, personable, and resilient.
Responsibilities:
Direct, guide, and assist Case Management staff to execute responsibilities of their position.
Design and manage Case Management patient care standards, policy, and procedure.
Ensure that Case Management staff consistently cooperate and collaborate with other hospital staff.
Understand, prioritize, and evaluate work undertaken in Case Management.
Coordinate Case Management services with needs of other services.
Assist patient family members with agency referrals for rehabilitative services, home health needs, and use of senior resources.
Serve as hospital liaison and representative in interaction with the community and other external entities.
Participate in Hospital committees to evaluate and assess needs of the Hospital, Nursing department, and Case Management.
Coordinate Hospital Utilization review according to established policy and procedure.
Via health information record review, assess, evaluate, and communicate with providers in regards to documentation quality.
Act as Hospital representative with insurance companies, maintaining awareness of policy benefit variations, providing appropriate clinical information to such companies, obtaining information about approved inpatient stay length, and communicating such information about stay length to Business Office staff.
Knowledge of disease management, healthcare reimbursement, and community resources
Qualification/Education/Requirements:
A BSN degree or higher from an accredited nursing school
Training and Experience as a registered nurse, at least 5 years.
Experience in case management, discharge planning, social services, utilization management
Management experience, at least 2 years
Current Oregon RN license
Possession of United States citizenship or proof of right to work in the United States

Pay: $83,200.00 - $114,400.00 per year
