Responsibilities:
The Nursing Assistant II will work a 12-hour varying shift and rotating weekends and perform patient treatments assigned by the Registered Nurse and other health care team members.
  • Assists with the unit's admission and discharge processes as evidenced by feedback from healthcare team members and observed by supervisor. Apprises the Registered Nurse and health care team of any observed significant changes in the patient's condition.
  • The Nursing Assistant II will communicate written and verbal information to health care team members as evidenced by feedback from healthcare team members and observed by supervisor.
  • Assist in training of Nursing Assistant staff on duties in the Department. Mentoring and monitoring Nursing Assistant staff at the direction of RN or Provider.
  • As a Nursing Assistant II, you will perform activities of daily living appropriate to patients condition.
  • Provides ADLs including, but not limited to, toileting, hygiene, monitoring intake and output, meal serving, setting up, and feeding patients, measured by patient and co-worker feedback.
  • The Nursing Assistant II will provide mobility activities including range of motion exercises, transferring, and ambulating of patients, as measured by direct supervisory observation. Establishes and maintains good communications with patients, families/visitors and hospital staff.
  • As a Nursing Assistant II, you will report vital information to all levels of the healthcare team as needed as evidenced by feedback from healthcare team members and observed by supervisor.
  • The Nursing Assistant II will participate in purposeful rounding and documentation of purposeful rounding as directed by Registered Nurse and per departmental policy.
  • Collaborates with Nurse Manager to support adherence to unit budget. Completes all unit specific and department competency requirements, as measured by supervisory review.
  • Attends relevant educational and training programs, as evidenced by signature on in-service attendance logs.
Qualifications:

Bassett Healthcare Network follows the New York State and New York State Department of Health COVID-19 vaccine mandates for healthcare workers. This mandate requires COVID-19 vaccinations for personnel working for or on behalf of our hospitals and nursing homes. The New York State mandate allows for medical exemptions and religious accommodations, when appropriate. All candidates who accept an offer from Bassett Healthcare Network will be required to show proof of vaccination as required by New York State.


Education
:

  • High school diploma or equivalent

Experience/Qualifications:

  • Minimum one year of previous direct patient care experience required and/or completion of one year of RN school from an approved ADN or BSN program.

Licensure/Certificates:

  • Certified Nursing Assistant Certification and/or EMT preferred but not required BLS or Heart Saver with AED must be obtained within 6 months from date of hire.


Patient Care Support Professional Nursing Pathway Requirement:

  • Outside Candidate - Completion of Application for Nursing Assistant Clinical Pathway and Unit based project by 6 month evaluations
  • Internal Candidate - Completion of Application for Nursing Assistant Clinical Pathway and Unit based project at time of application.

Skills:

  • Reading, writing, typing (at least 20 words per minute), and basic math skills required.
  • Ability to communicate effectively with patients and staff.
  • Ability to function as an active team member.
  • Demonstrates competency in required skills relevant to area of practice.


Physical Requirements:

  • Walking, standing, bending, and kneeling throughout the shift.
  • Ability to lift and position patient is required.
  • Moving of heavy, awkward equipment.
  • Category I risk exposure as defined by OSHA.
  • Must be able identify and differentiate colors
