We are looking for a compassionate & team-oriented Nurse Practitioner to work in our In-Patient Palliative Care Consult program.
In this role, our Palliative care APRN would work in collaboration with physicians, APRNs and other palliative care team members, in an in-patient consult service to deliver palliative care and related health care services to patients and families. In this role, Palliative care APRN is responsible and accountable for the clinical management of the adult patient appropriate for palliative care and for carrying a patient caseload in collaboration with the palliative care team. The APRN is expected to assess/refer patients for both their physical and psychosocial needs. Our APRN would also have the opportunity to provide routine palliative care teaching to patients, family members, Nurses and Residents.

Qualifications
- Licensed to practice in state of Connecticut
- Master's Degree in Nursing required
- Certified as Nurse Practitioner required
- DEA and CT Controlled Substance Registration
- Works well with a multidisciplinary team.
- Able to work independently.
- Excellent communication, clinical and interpersonal skills to facilitate patient care
- 1 year experience as an APRN, preferably in Palliative Care, Hospice Care and/or Oncology settings. Will consider new APRN graduates.
