We are hiring an Intake Nurse to manage intake and referrals for our Home Health team and supervise intake team members.

As the Intake Nurse for our Home Health team, you will:

  • Receive and triage home health and hospice referrals from all sources via phone and email.
  • Explain home health and hospice services to phone callers including physicians and other health care referral sources and clients, family members, and caregivers.
  • Ensure documentation and orders are in place; monitor required documentation ongoing.
  • Complete, maintain, and submit accurate and relevant clinical notes regarding intake/referral information using the electronic medical record (EMR).
  • Communicate with referral sources, physicians, and clients via phone and email about referral status.
  • Refer to other WesleyLife programs and community resources as necessary.
  • Maintain relationships with primary referral sources by meeting with organizational leaders and collaborating to develop transitions of care policies, processes, and workflows.
  • Collaborate with the sales team and clinical managers to increase referrals received and review discharges to meet budgeted census goals.
  • Manage Intake Coordinators and Insurance Verification Specialist including hiring, addressing performance issues, terminations, performance reviews, timekeeping, and other supervisory tasks.
  • Travel in all weather conditions and to all areas of the home health service area to perform co-visits or client visits as needed. May participate in a home health on-call rotation for nights and weekends.
