As the Intake Nurse for our Home Health team, you will:
- Receive and triage home health and hospice referrals from all sources via phone and email.
- Explain home health and hospice services to phone callers including physicians and other health care referral sources and clients, family members, and caregivers.
- Ensure documentation and orders are in place; monitor required documentation ongoing.
- Complete, maintain, and submit accurate and relevant clinical notes regarding intake/referral information using the electronic medical record (EMR).
- Communicate with referral sources, physicians, and clients via phone and email about referral status.
- Refer to other WesleyLife programs and community resources as necessary.
- Maintain relationships with primary referral sources by meeting with organizational leaders and collaborating to develop transitions of care policies, processes, and workflows.
- Collaborate with the sales team and clinical managers to increase referrals received and review discharges to meet budgeted census goals.
- Manage Intake Coordinators and Insurance Verification Specialist including hiring, addressing performance issues, terminations, performance reviews, timekeeping, and other supervisory tasks.
- Travel in all weather conditions and to all areas of the home health service area to perform co-visits or client visits as needed. May participate in a home health on-call rotation for nights and weekends.
