  • Position Summary

    Under direction of the Program Manager of Adult Mental Health and within federal, state and local guidelines performs routine case management work. Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs.

    This position is responsible for overseeing the care of individuals served assigned to various Levels of Care (LOC). Works to ensure the continuity of care for individuals served. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.



    Essential Duties
    • Attends and participates in all scheduled team meetings.
      • Staff will attend meetings as scheduled and sign in for meetings to acknowledge their presence and understanding of the topics discussed.
      • If a meeting is missed, staff will get with supervisor within one week to review the information.

    • Provides courteous, professional services at all times.
      • Maintains professional boundaries.
      • Interacts with Center staff, individuals, and community members in a professional manner.

    • Provides Case Management services.. Caseload size will vary.
      • Provide supported employment, supported housing, budgeting, skills training as appropriate, etc., to individuals served as defined in Texas Resilience Recovery (TRR) guidelines.
      • Provides resources/referrals to meet individual needs and helps them navigate different systems.
      • Will assist other teammates in their absence by providing services to individuals on their caseload as needed.

    • Completes community base visits per minimum monthly contact.
      • Community based visits to occur 20% of the time for individuals approved for Level of Care (LOC) 1 and LOC 2.

    • Provides transportation for individuals as needed.

    • Safely transports individuals using Center vehicles and/or personal vehicle without accident, citation or another incident.
    • Follows all Center guidelines while driving, including restriction of food, drink, smoking, and cell phone use.
    • Completes all required vehicle checklists as scheduled.
    • Maintains fuel and fluid levels of vehicles and reports all maintenance issues immediately to appropriate supervisor.
    • Maintains personal driving record within agency standards and requirements.
    • Provides ongoing evidence of current auto liability insurance.
    • Provides transportation in own vehicle only as approved by supervisory staff.
    • Maintains his or her personal vehicle and company vehicles to ensure the safety of individuals and other staff.
    • Completes vehicle paperwork in a timely manner.
    • Keeps company vehicles clean and disposes of trash and clutter daily.
    • Drives safely and adhere to all Center and state laws.

    6. Ensures each individual served is provided services as required by the level of care assignment.

    • Engagement measure with consumers is at 65.6% on a monthly basis.
    • Maintains contact standards as defined in TRR guidelines.

    7. Administers clinical assessments and paperwork to determine appropriateness of level of care assignments and level of rehabilitative services required to meet individual’s needs.

    • Required paperwork includes: Adult Needs and Strength Assessment (ANSA), Recovery Plan, Substance Abuse Needs Inventory, Rights Review, TIMA consent forms, Tobacco Cessation Form, and Columbia-Suicide Severity Rating Scale (CSSRS) Form completed prior to expiration and as needed based on individual change.

    8. Is familiar with all applicable Texas Administrative Code (TAC), fidelity tools, and Texas Human Health Services (HHSC) contract information and Center policies and procedures.

    • All documentation is completed within two business days, to meet or exceed 95% monthly.
    • Skills training notes include fidelity words.

    9. Ensures individuals charts are in compliance with HHSC contract requirements and standards.

    • Responds to all chart audits by assigned deadline.
    • All paperwork will be signed in black ink/or electronically in the EHR with credentials and date.
    • All paperwork is completed within Center policies and procedures.

    10. Provides emergency/crisis intervention when necessary according to HHSC and Center policies and procedures.


    11. Completes other duties as assigned.

    • All assignments given are completed in a timely manner.
    • Has the ability to work independently and make decisions but seeks guidance when needed.




    Education and or Experience

    Graduation from an accredited four-year college or university with major course work in a human services field as required to qualify as a Qualified Mental Health Professional (QMHP) and six (6) months prior experience in a mental health field preferred. Master’s Degree in a human services field and previous Crisis experience preferred. English/Spanish bilingual preferred.



    Registration, Certification, Licensure & Qualifications
    • Individual needs to meet the requirements to qualify as a QMHP (Qualified Mental Health Professional).
    • All job specific training is required to be completed within 30 days of hire.
    • This position requires a valid Texas driver's license, auto liability insurance and a driving record acceptable to the Centers insurance requirements.
    • The position requires adherence to all applicable laws, regulations, policies, procedures and/or guidelines of HHSC and DCMHMR. Other specific laws, regulations, policies, procedures and/or guidelines which must be followed include: Texas Administrative Code and Case Management Guidelines.
