About Your Work

The Manager of Quality and Care Management is responsible for improving the health of the overall population of FCS patients. The Manager will implement and coordinate the FCS medical group performance improvement program including the Value-Based Pay-for-Performance (VB-P4P) program, Medicare Star Program, Medicare MIPS and post-acute care QI projects according to prescribed goals and policies. The Manager also has responsibility for chronic care management. This includes responsibility for population health management with special attention to patients with complex conditions. The position requires the ability to effectively manage multiple projects, collect and evaluate data, generate reports and communicate between internal and external departments.

You Will Focus On:

  • Care Management: Monitoring Transitional Care Managers and Chronic Care Managers and ED utilization
  • Mentoring of the care managers to problem solve complex patient needs and care challenges
  • Partner with Medical Director to build care management capabilities within the organization
  • Value-Based Pay for Performance, Medicare Stars, and other quality initiatives
  • Attend monthly FCS/Altais QIC, the monthly QI/IT Committee and the QI Sub-committees at each of the four FCS sites (Facilities) and present quality-related information at such meetings.
  • Work with Medical Director to update the annual Quality Improvement Program description, work plan and annual evaluation. Work with Medical Director, IPA Medical Director and Aledade to complete semi-annual reports and submit to the FCS/Altais Quality Management Committee on a timely basis as well as assist in the development, improvement and implementation of policies and procedures related to quality management.
  • Develop disease registries and i2i gap analysis to prepare and disseminate reports to providers using HEDIS, Integrated Healthcare Association (IHA) value-based P4P metrics (A.M.P.), Medicare Stars, Medi-Cal Accountability Sets (MCAS)and Aledade CMS MSSP and commercial ACOs guidelines as directed by the FCS Medical Director.
  • Support the implementation of chronic disease management programs as directed
  • Work with FCS Facility Medical Directors to implement VB-P4P, Medicare Stars, and CMS MSSP. Commercial ACO and MCAS requirements. Work with Facility Medical Directors to coordinate annual wellness visits (AWV), Patient Annual Health Assessment (PAHA) and Hierarchical Condition Codes (HCC). Assist in alignment of Chronic Care Management (CCM), Transitions of Care (ToC), Complex Care Coordination (CCC) and Advanced Care Planning (ACP) services for the elderly
  • Work with Office Managers to utilize FCS IPA, Optum/HealthCare Partners IPA, CareMore, and Global IPA information portals to develop performance reports
  • Foster positive interaction and relationships with all internal departments as well as cultivating positive working relationships with external contacts.
  • Prevent and avoid harassment and discrimination. Respond promptly to any complaints in accordance with policy in employee handbook
  • Perform other assigned duties / special projects on an as-needed basis.
  • Attend monthly PCMH team meetings

The Skills, Experience & Education You Bring:

  • Minimum 5 years’ experience in clinical practice and/or health administration; licensed clinician preferred
  • Minimum of a Bachelor’s Degree in Public Health, Health Administration, Business, or clinical experience.
  • Strong computer skills (electronic medical record and related software, word processing, spreadsheets, database access, analysis and reports).
  • NP or PA Certification
