Position Summary*
Responsible for providing culturally sensitive health education, parenting workshops, male involvement activities, and case management to fathers/male partners, including assessment, referral, follow-up, monitoring and needed supportive services. Plan and attend community outreach events with the purpose of recruiting fathers for services. Services are provided during regular business hours as well as evenings and weekends as needed.
Essential Functions

• Provides case management, including assessment, referral, follow-up and monitoring to fathers and male partners, through routine home and office visits and regular phone contact.
• Supports male clients in planning for their own health and social service needs by assisting clients in identifying goals.
• Facilitates parenting and life skills workshops for fathers/male partners
• Conducts health promotion activities by providing client education, disseminating educational literature, and distributing disease prevention materials including condoms.
• Refers participants needing more intensive follow-up to appropriate agencies. Learns about and makes contact with community resources needed by clients.
• Conducts weekly community outreach activities to identify, recruit and enroll eligible clients.
• Plan and/or coordinate program presentations and community outreach events to create awareness about father inclusion and service utilization.
• Assists in organizing client meetings and other special events.
• Facilitates community health education and male involvement activities.
• Attends community meetings, health fairs and represents MCC as requested.
• Keeps careful records of all contacts made and follows up as necessary. Keeps accurate records of services received by clients.
• Attends orientation and ongoing training sessions on maternal and child health and welfare, patient navigation, child abuse, housing, nutrition, substance abuse, counseling, parenting, infant and family support issues.
• Meets with supervisor regularly to report on and discuss work.
• Ensures confidentiality of client records, sharing pertinent information only with the client’s approval.

Experience, Education, and Licensure

• Bachelor’s degree in Social Work, Public Health, or related field strongly preferred.
• Experience with community health issues; fatherhood/male engagement; community outreach; and maternal and child health preferred.
• Experience providing case management services.
• Commitment to mission of MCC and Healthy Start.
• Bilingual preferred
• Valid driver’s license

Job Type: Full-time

Pay: $39,000.00 - $43,000.00 per year
