Job Title: LPN-SA
Requirements: : a Licensed Practical Nurse with any combination of education (above high school level) and / or experience and training equivalent to 5 years in mental health, substance abuse and co-occurring disorders fields utilizing the knowledge, skills and abilities listed above and associated with such positions as clinician, counselor, therapist, social worker, psychologist or psychiatric nurse. Possession of a valid Virginia or North Carolina driver's license. COVID VACCINATION REQUIRED.

Specific Credentials (Licensure/Certifications): Current unrestricted Virginia LPN license.

Location: Virginia Beach, VA
Hours: 7am to 3:30pm or 8am to 4:30pm
Start Date: ASAP
Hourly Pay Rate: $23.18 per hour

Summary: Provide medical / clinical services to an assigned caseload comprised of adult clients with mental health, substance abuse or co-occurring disorders who are receiving medication management services. Provides services to a caseload of PACT consumers and works closely with other members of the PACT team. Designated at PACT site Medication Room Nurse, responsible for following the guidelines necessary to maintain the Virginia State Board of Pharmacy Controlled Substance Registration and ensuring compliance with Medication Room policies.


Duties: Performs nursing duties to include, but not limited to assessing/checking-in consumers in preparation for physician visits, administration of oral and IM medications, transcription of physician orders, ordering and packaging medications, and providing education/counseling regarding medications and relevant nursing problems.

Transcribe medication orders to the MAR and provide oversight of MAR completion to ensure accuracy and avoid errors in the program. Provide guidance to teammates as appropriate to correct MARs.

Assists in the provision of medical services, such as delivering and monitoring medications, scheduling and accompanying consumers to medical appointments, collaboration with medical providers and requesting medical records.

Performs comprehensive assessments for persons with chronic mental illness and co-occurring disorders to determine service, supports and other needs to assist consumers with community stability. Collaborates with team in developing treatment plans with participation of the consumer and/or Client and facilitates clinical case reviews. Completes all required clinical documentation within agency standards.

Provides linkage, monitoring, and coordination of services, including medical, social, educational, housing, benefits, etc. Assist consumers with attending necessary appointments in the community to ensure needs are met.

Supports team by collaborating in developing emergency assessment, treatment, and support during work hours and via the PACT emergency phone rotation. Where appropriate, conducts or assists with TDOs and is present at hearings for PACT consumers.

Provides supportive counseling in 1:1 and group settings to assist consumers with the development of improved health/wellness, coping, problem-solving and communication skills related to treatment goals.

Provides training and support with activities of daily living, performs crisis intervention, and facilitates prescreening/hospitalization when necessary. Assists and participates in all hospital diversion, hospital care coordination, and hospital discharge/re-integration services.

Establishes and maintains inter-agency working relationships regarding the provision PACT services and other supportive services. Collaborates with other providers, organizations, agencies, etc. that are involved with the care of PACT consumers.

Maintains clinical records in accordance with regulatory standards and ensures protection of consumer rights and confidentiality. Utilizes an electronic health record.

Abides by all policies and procedures and adheres to standards of service.

Participates in team meetings, clinical staffing and supervision (individually and in group).

Participates in professional development training, seminars, and conferences.

Performs other related duties as requested or assigned.

Virginia's Board of Nursing Laws and Regulations and applicable State, Federal, and agency standards, including Board of Pharmacy regulations
Human Rights
Psychiatric, Substance Abuse, Co-Occurring disorders and related medical conditions
Psychotropic medications including newer atypical antipsychotics; indications for use, side effects, dosages and routes of administration
OSHA standards; manages hazardous waste appropriately
Medical conditions related to mental illness
Principles and practices of holistic nursing care
Tardive Dyskinesia screening using AIMS examination
General Nursing skills (BP, weights, BMI, etc.)

Skills:
Assessment of blood pressure, body mass index, mental status examination, screening for medication side effects, and Tardive Dyskinesia screening using AIMS examination
Medication administration, including injectable antipsychotic medications
