The Palms of Sebring is seeking a Licensed Practical Nurse to join the team our growing team!

The Palms of Sebring located in beautiful downtown Sebring, Florida offers Assisted Living, Independent Living, long term care, and Memory Care. This campus offers care for 276 residents and patients who rely on us for life enrichment and quality of care.

Your Role in our Company

The LPN is responsible for rendering nursing care in assigned area and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing. This position is a part of a team on a dedicated unit.

Essential Elements of your Position

  • Utilize nursing knowledge and skills in the safe implementation of basic preventative, therapeutic, and rehabilitative nursing care of assigned resident as evidenced by documentation and observation of positive resident care outcomes.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards.
  • Contribute to the development of the resident’s plan of care based on the identified resident needs.
  • Assist in the evaluation of the effectiveness of the nursing care administered relative to the resident’s identified needs.
  • Document pertinent observation and intervention in the electronic health record.
  • Perform selected tasks related to patient care.
  • Administer medication accurately, observing resident response, as documented in the medical record. Perform limited I.V. Therapy procedures on those residents receiving peripherally inserted Intravenous fluids, as delegated and observed by the RN.
  • Perform limited I.V. Therapy procedures upon proof of successful completion of Intravenous Therapy course, as observed by Department Director and RN.
  • Maintain and coordinate communication among residents, staff, and physician as evidenced within the medical record.
  • Assure effective communication with patient/family as reflected in the plan of care.
  • Give punctual change of shift report consisting of pertinent and relevant information for assigned patients.
  • Effectively communicate patients changing needs.
  • Work collaboratively with other members of the health care team in coordination of the patient’s care.
  • Provide patient/family education.
  • Demonstrate flexibility to perform duties wherever volume deems it necessary with the Organization.
  • Assist residents, family members or others with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
  • Answer and refer telephone calls or other inquiries to ensure accurate and timely communications are facilitated. Identify yourself in a pleasant and positive manner. Take responsibility for helping the caller.
  • Assist others in the performance of their assignments.
  • Recognize the need for variations in staffing and volunteer to fill open shifts.
  • Take initiative to do or redo inadequate or incomplete work, even if it is not yours.
  • Ensure compliance with regulatory standards.

Productivity/Efficiency

  • Consistently demonstrate ability to take the initiative to make decisions/choices without direct and ongoing supervision.
  • Participate in continuing education activities and obtain required contact hours.

Qualifications:

Currently licensed LPN.

Minimum of one year of experience preferred but no required familiarizing with Assisted Living or Skilled Nursing a plus.
