Integrated Care Provider – CCBHC Grant – Posting #22-145
Major Duties:
This position works under the supervision of the Program Director and is responsible for providing comprehensive primary care or referred as integrated care. Maintaining compliance of both programs with all applicable counties, state and federal codes Responsibilities include:
- Engage in a collegial consultative relationship with other primary care providers and mental health practitioners and as needed with registered nurses, case managers, residential treatment center staff, and outpatient treatment center staff.
- Work as team member in a medical home model with other primary care and behavioral health staff.
- Provide consultation if requested by other programs in the agency or by the agency as a whole.
- Review patients’ charts and maintain up-to-date record of allergies, medical problems, social history, mental health history, SUD history, medications, and immunization status.
- Request and review prior medical history and management as is relevant to ongoing primary care.
- Provide comprehensive primary health care services including health promotion, prevention of disease and disability, health maintenance, rehabilitation, identification, diagnosis and management of health problems, and referral to other providers as appropriate.
- Provide MAT evaluation, treatment plan and medication management
- Work in integrative care model with behavioral health
- Evaluate, examine, and provide individualized treatment plan for acute and chronic issues, and minor injuries for patients.
- Order and interpret diagnostic tests including but not limited to labs and imaging.
- Treat superficial lesions such as warts, cysts, minor lacerations, abrasions, minor first and second-degree bums within the provider’s scope of practice.
- Treat minor injuries such as strains, sprains, and non-complicating extremity fractures and dislocations within the provider’s scope of practice.
- Counsel and instruct patients in health maintenance problems such as: 1) nutrition, diet and weight reduction; 2) risk factor prevention and modification (chemical addictions including alcohol, tobacco and drugs); 3) mental health referrals; 4) self-care measures for health maintenance; 5) family planning.
- Prescribe medical treatment and medications.
- Provide up to date recommendations and administration of vaccines.
- Identify and appropriately refer patients to higher level of care in a timely manner.
- Assist medical team in coordination of care that the patient receives from other providers and agencies and advocates for the patient within the health care system.
- Develop and document plan of care and outcomes evaluation in a timely fashion.
- Review medical histories and make referrals to other agencies and providers as appropriate.
- Maintain commitment to providing culturally-sensitive care.
- Assist the medical team with health education to individual patients or groups as needed.
- Provide staff development education as requested by the Program Director.
- Engage in ongoing education and specialized training in order to maintain a high quality of care to the clinic's patient population.
- Attend staff/provider meetings.
- Document patient care legibly and appropriately within medical standards of care.
- Notify Human Resources regarding any changes in state licensure, DEA licensure, board certification, Medicare provider status; send updated copies of licenses to human resources.
- Participate in periodic peer review of patient charts.
- Supervise and train new incoming providers, including mid-levels: NP and PA.
- As per supervision regulations of PAs, review required amount (identified by state PA supervisory guidelines) of patient encounters done by provider assistant, assessing and ensuring PA treatment plans and care is achieving appropriate level for his/her position.
- Perform other duties as assigned.
Requirements:
- Washington State Department of Health ARNP, PA or MD License required;
- Board certified or Board Eligible in Family Practice required;
- CPR card;
- Possess a current DEA credential with X waiver;
- Experience working with patients who have behavioral health conditions preferred;
- Ability to maintain patient confidentiality;
- Good verbal and written communication skills;
- Great attendance; and
- Ability to work well with others
