Infectious Disease Care Coordinator Job at ASCENSION DEPAUL SERVICES
Serves as the content expert to all associates of the clinical teams to improve ID patient care services.
Assists in providing administrative oversight of Infectious Disease services.
Assists ID team with creating and revising workflows for ID patients in various programs.
Keeps abreast of information related to Infection Diseases such as Hepatitis C and HIV.
Serves as an active member on the Infection Control and Prevention committee. Performs five hand hygiene observations per health center per month and submits hand hygiene observation forms each month for timely reporting.
Prepares and submits monthly reports for each condition including but not limited to Hepatitis C, HIV and PrEP.
Serves as an expert resource for ID services and supports efforts to promote Hepatitis C services for patients within DCHC health centers.
Arranges schedules for initial evaluations and the appropriate follow up. Collaborates with Patient Access teams to arrange transportation as needed.
Responsible for initiating and completing Prior Authorizations for patients ready for treatment at all DCHC sites. Works closely with the Pharmacy team to ensure the timely completion and submission of Prior Authorizations for patients starting treatment.
Coordinates the Peer to Peer appeals and ensures that timelines are met to obtain medication in timely manner.
Calls patients when medications arrive. Instructs the patient on the importance of taking medications and adherence.
Oversees tracking of patients on therapy and follows up as needed.
Places follow up calls to patients and track their status to ensure patients are scheduled for follow up visits with a goal to complete therapy. Updates logs at least weekly.
Ensures that follow up on patients that need Hepatocellular carcinoma screening was completed.
Completes follow up calls when patients miss appointments
Ensures that the patient is completely prepared for the provider’s visit
Ensures that patients have completed all labs and diagnostic tests as needed
Oversees ID services patient schedules including labs schedules and other schedules for ID providers during and the upon completion of therapy.
Communicates daily with providers on the progress of patients and ID services.
Attends all Hepatitis C clinics on designated days at the various Health Center to ensure smooth operations of clinics.
Attends all meetings related to ID services, general staff meetings and other essential meetings.
Communicates with Pharmacists, RN Care Managers at each site, and providers regarding challenges, barriers and successes of obtaining information, medication, etc.
Ensures that certified letters are sent as needed to communicate with patients.
Attends trainings and inservices to garner learning of Hepatitis C.
Identifies patients for which PrEP would be beneficial including patients with positive Sexually Transmitted Infections, Multiple STI testing, Men Sleeping with Men.
Oversees the Navigation Services for patients on PrEP and ensuring that every aspect of care has been addressed.
Provides patient education for PrEP.
Establishes referral relationships with local Emergency Departments, promoting DCHC for PrEP care.
Collaborates with DCHC Development Department to identify funding sources and complete grant requests.
Coordinates linkages to care for newly diagnosed HIV positive patients. Works with patients to arrange appointments within one week of diagnosis.
Tracks HIV positive patients and ensures patients are connected to the appropriate resources.
Ensures that CD4/Viral load/ART are documented in the patient’s EMR.
Coordinates follow up appointments for DCHC patients with positive Tuberculosis tests.
Minimum five years of experience in healthcare (Bilingual individuals are encouraged to apply)
Must have personal and reliable transportation, valid Driver’s license, Minimum current liability automobile insurance required
