Provides case management for individuals and families who are experiencing or are at risk of homelessness or housing instability. Works with the customer to identify barriers to housing stability and refers to appropriate programs, resources, and partner agencies to help become stably housed or maintain stable housing.

$24.80 - $31.41 per hour plus benefits (see benefits tab on the job posting). The position will normally start at $24.80 per hour.

Essential Duties and Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Provides responsive customer service to engage individuals who are experiencing housing instability and are interested in participating in the housing case management program. Once customer is enrolled into the program, the case manager maintains regular contact, to include in-person appointments at the Department, visits to participant households, or appointments scheduled at community locations convenient to the customer.

Meets with participants to evaluate needs, assess strengths, and work together to build support around long term housing stability. Develops an easily understood individualized plan using family strengths and resources to resolve barriers. Identifies strategies to address any identified short-term or chronic issues, such as substance use or mental illness, which might compromise achievement of goals and trigger relapse into homelessness.

Develops and maintains a working knowledge of all community resources and services to include public assistance and other WCDHS programs. Assists with referrals for services to help customers obtain or maintain stable housing. Advocates with agencies, landlords, and other community organizations to support customers working on housing stability.

Provides compassionate, professional, and innovative case management services by sharing knowledge-based information and resources with participants. This includes providing support around budgeting, life skills, tenancy skills, and effective communication.

Collaborates with coworkers and program leadership to evaluate program and service accessibility and impact to identify gaps and areas of need.

Evaluates customer needs on an ongoing basis, adjusting and updating individualized plans accordingly. Shares pertinent information with community partners within the bounds of confidentiality and when appropriate, collaborates with them in housing stability planning. Follows up with participants and partners about agreed upon activities in a timely manner. Maintains communication with service providers and participants to ensure responsible and effective use of resources.

Accurately establishes and maintains client files in accordance with Department policy and appropriate local, state, and federal regulations. Documentation includes all required contact notes, progress reports, supportive service documentation, and partner agency updates. Tracks and monitors participants activities by reviewing and entering information accurately and timely.

Qualifications

MINIMUM QUALIFICATIONS

Required Education:

  • High School Diploma or equivalency.
  • Successful completion of an associate degree is preferred.
  • Equivalent combination of education and experience may be considered in lieu of degree.

Required Work Experience:

  • Two (2) years full time business or clerical experience in an office environment which includes extensive customer service and proven experience working with the public.
  • Experience communicating both verbally and in writing with a diverse group of individuals.
  • Experience in database management, data tracking and outcome reporting.
  • Experience using Microsoft Windows, Word, Excel, and Outlook.

Preferred Skills and Abilities:

  • Experience in a public or private social services agency.
  • Experience working with community stakeholders.
  • Experience working with low-income populations.

Required Skills, Abilities and Knowledge:

  • Must have demonstrable skills in critical thinking and independent judgment.
  • Must be able to exercise discretion, analytical skill, judgement, and personal accountability.
  • Must be able to use discoveries and interpretations to improve data, methods, and outcomes.
  • Must be able to adhere to work product deadlines.
  • Must possess ability to keyboard at 35 words per minute.
  • Possess a valid Colorado Driver’s License and Liability Insurance. Must provide own transportation and be willing to travel throughout the State. Must be willing and able to travel to other states on occasion when needed.
  • Must pass Human Services Background Checks. Background checks include CBI/FBI fingerprinting and will continue to be monitored throughout employment.
Other Necessary Requirements

ADDITIONAL REQUIREMENTS

Ability to enter data into several different computer programs. Ability to learn and apply new computer skills on a regular basis. Knowledge of computer programs such as Microsoft Windows and Word, Excel, PowerPoint, Teams, OneNote, and Outlook.

Ability to read and comprehend instructions, correspondence and department policies and procedures. Ability to write correspondence.

Ability to effectively communicate in one-on-one and small group situations to customers, and other employees of the organization.

Ability to apply math concepts and calculations in the work environment.

Ability to apply common sense to problem solve in a work environment.

Ability to handle high stress situations in a fast paced, high volume and ever-changing work environment and be able to handle high stress situations.

Follow Human Services policies and procedures, the Weld County Code of Conduct and adhere to strict confidentiality guidelines and appropriate release of information outlined in both.

Work may involve time away from a standard office environment, which may consist of driving to and from home visits. Work may occur in situations in client's homes, which could be considered a hostile or unsanitary environment. Work office will be on the Greeley Campus, but employee may perform work at one of the other satellite offices when required.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee may be required to sit for extended periods of time and is occasionally required to stand and walk. Occasionally the employee may be required to lift and/or move heavy files or objects up to 25 pounds. The employee frequently is required to use their hands and reach with hands and arms. Specific vision abilities required for this job include close vision, distance vision, peripheral vision, and ability to adjust focus such as that used when using a computer or reading.
