Position Info: :
- - $28.85 - $31.00 per hour
- Must be willing to work directly with homeless individuals and patients may have been tested positive for COVID-19 (or awaiting test results):
- Minimal contact with individuals and personal protective equipment (PPE) is provided
- Temporary, full-time, nonexempt positions, not eligible for benefits

How You Make an Impact:
- Support hotel guests who have tested positive for COVID-19 (or awaiting results) and homeless individuals.
- Link participants with services for mental/physical health, housing, substance recovery, and other services
- Provide initial intakes and Coordinate Entry assessments
- Support guests with obtaining their Health and Housing documentation for document readiness
- Help identify potential housing opportunities, complete paperwork, and prepare for housing
- Support move-in to new unit, interview with landlords, and unit viewings
- Utilize motivational interviewing techniques to explore participants’ substance abuse using Harm Reduction Model
- Compile data, prepare reports, and maintain thorough and concise case notes
- Participate in weekly case management and other program meetings
- Respond to client requests and escalate as necessary
- Serve as on-site liaison with county health care staff
- Communicate any issues to leadership, and/or with property owners as authorized by leadership
How You Meet the Qualifications:
- MA/MSW preferred. BA/BASW required. 2+ years case management experience with homeless to qualify
- Experience working in an outreach, shelter, drop-in, or residential setting preferred
- Must have 1 year of field experience working with people with serious mental illness, dual diagnosis and/or the homeless population
- Those with lived experience or who are bilingual are highly encouraged to apply
- Demonstrated sensitivity, cultural competency, professional boundaries, and ability to build supportive, respectful working relationships and instill hope and promote self-determination
- Ability to engage others easily, maintain an empathetic stance, and de-escalate situation
- Understand the special needs of the homeless population
- Basic computer skills, proven ability to work effectively as an individual and part of a team, with minimal supervision
- Must have own car to travel throughout the county, valid California Drivers License, auto insurance, and clean DMV record required.
- Flexibility with work schedule. Ability to work flexible hours, including weekends and evenings.
