Responsibilities + Duties

• Work with a registered nurse and other clinicians to help care for clients in their homes or designated facility according to the service plan
• Provide companionship and personalized care while encouraging independence and self-care
• Assist clients with activities of daily living, including dressing, bathing, grooming, toileting, and eating
• Assist clients with self-administration of medication as needed
• Provide mobility and transfer assistance within guidelines
• Prepare meals and snacks which may include grocery shopping
• Perform light housekeeping, including vacuuming, laundry, and linen changes
• Assist with incontinence care
• Assist with transportation needs
• Remain in compliance with HIPAA guidelines as well as all state and federal regulations.
• Document clinical notes in a clear and concise manner.

Qualifications + Skills

• High school diploma or its equivalent
• CPR certification
• Ability to lift at least 50 pounds
• Reliable transportation
• Valid driver’s license and insurance
• Prior experience as a caregiver or working with seniors preferred
