Reports to: Sexual Health Services Manager

Work Site: Bellmore, Nassau County

Responsibilities:

1) Provide community and social media-based outreach to young gay and bisexual men and transgender women (ages 13-30) throughout Nassau and Suffolk Counties, Long Island, with a focus on African American and Latino populations for PEP/PrEP services and HIV/STI testing. Community outreach venues will include local colleges, bars, nightclubs, street/neighborhood locations etc.. Social media outreach will include Facebook, Twitter, Tumblr, and dating apps such as Grindr and Jack’d. Outreach will include tabling, giving out safer sex packets, and providing education on PEP/PrEP and HIV testing services.

2) Provide confidential HIV and STI testing and counseling for high-risk individuals during programming, via mobile testing services, and by appointment, with a particular focus on young gay and bisexual men and transgender women of color. Maintain all HIV testing logs, equipment, and records.

3) Utilize electronic health record system to coordinate and schedule all express STI testing appointments which includes conducting insurance verification

4) Conduct PrEP/PEP evaluations with high-risk individuals with a particular focus on young gay and bisexual men and transgender women of color. This includes coordinating linkage into medical services with ongoing monitoring and support for all PrEP/PEP support program enrolled clients.

5) Identify, screen, and link new PrEP/PEP clients monthly to agency’s HNS/PrEP support programs.

6) Provide comprehensive sexual health testing/screening services at agency-sponsored events and at community-based events/venues (bars, clubs, parties).

7) Coordinate linkage to STI testing as well as ongoing medical care for newly diagnosed HIV positive clients.

8) Promote the initiation, retention in, and adherence to, client-centered sexual health based medical interventions.

9) Maintain all required electronic and written records of testing, linkages and outreach activities, in addition to maintaining monthly statistics of clients served.

10) Additional activities as assigned by the Sexual Health Services Manager.

Requirements:

Some college preferred, high school diploma or equivalent is considered. Experience (work or lived) with the LGBTQ community, including HIV testing and prevention work, along with individual counseling experience a plus. Bilingual English/Spanish is a strongly preferred. Good written and oral communication skills. Work schedule requires both evening and weekend availability. Ability to manage time well and handle multiple tasks. Knowledge of Microsoft Office. Travel throughout Nassau, Queens and Suffolk Counties is required – must have own transportation (car and driver’s license) within one year of employment.

Job Type: Full-time

Pay: $32,000.00 - $36,000.00 per year
