The Foster Care Homes Worker uses his/her abilities, under supervision, to assure that licensed foster families are considered and assessed for possible placement, and supports foster families in providing temporary safe and stable care for children in out-of-home placement until permanency is achieved.

• Foster Care Homes Worker engages and assists foster families for placement of children and provides necessary supports for successful placement.
• Foster Care Homes Worker completes written annual documentation and licensing requirements, under supervision, per DCF Regulations for Foster Family Homes
• Foster Care Home Worker coordinates and conducts TIPS-MAPP and Deciding Together training for prospective foster families.
• Foster Care Homes Worker provides training regarding foster care, as needed
• Facilitates and assures that conferences or staffings and related activities are conducted within standards
• Completes and distributes written documentation accurately and timely
• Liaison with DCF, the courts, community resources, and families
• Provides services according to compliance guidelines
• Participates in team meetings
• Participates in on call rotation after business hours.
• Provides excellent customer services to all internal and external stakeholders.
• Implements SFM policies and follows directives as required. Follows and adheres to all pertinent SFM Standard Operating Procedures (SOP’s), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and contractual terms
• Is knowledgeable of and follows all safety procedures
• Reports unusual incidents through appropriate Risk Management, clinical and safety channels
• Ensures clients’ rights are protected.
