Characteristics of Work

The primary responsibilities of this position include advocating and working primarily with unable-to-work families from every station of life, and sometimes dire economic backdrops, to ease them through transitions, hardships, and mentally tough times. This work also involves assisting families in finding the proper opportunities to obtain the resources or supplements needed for stabilization. The incumbent will also serve as a liaison between families and the family’s needs. Other duties include interviewing, counseling, referrals, developing and maintaining case files and case management, and other related activities in providing services to eligible applicants under the various programs provided by the Agency.

Essential Functions

These essential functions include, but are not limited to, the following:

Completes intake and eligibility procedures for primarily elderly and disabled applicants of Agency direct services.

Determines nature of customer’s situation by interviewing client; assessing social information to identify applicants’ personal strengths and needs and translate them into a set of realistic short term needs.

Documents appropriate service plans with customers to include establishment of ability to achieve needs using the Child Outcome Planning and Administration (COPA) and other mandated programmatic systems. Records detailed case notes to provide on-going documentation of the customer’s movement through the program and evidence of the strategies and resources used to impact the desired outcomes.

Provides information and referral services to persons needing assistance from other social service agencies. Complete Head Start, Low Income Home Energy Assistance Program (LIHEAP), Community Services Block Grants (CSBG), Emergency Solutions Grant, City of Jackson Emergency Services Program, CMPDD Transportation and other applications.

Ensure electronic records of families are maintained and up to date including, but not limited to, requirements for: CSBG Standards; Head Start standards as it relates to eligibility, recruitment, selection, enrollment, attendance, family, health, and nutrition services; and LIHEAP requirements. Maintains accurate and up-to-date case files. Ensure files are maintained in one central location and filed in alphabetical order as well as all sites required under program policies.

Maintains documentation of services provided through entries in COPA and mandated programmatic systems. Monitors and/or follows-up on applicant’s progress periodically, as prescribed and/or required. Enter and update all customer data and follow up information in accordance with Standard Operating Procedure (SOP) and enter into COPA database.

Ensures families have access to all needed resources both internally and externally. Ensures on-going relationship building with families. This could include conducting home visits to ensure opportunities are met.

Compiles and submits target reports on the services provided to or received by the customers; and/or, on the customer’s outcomes in the program to the supervisor as scheduled. Reviews and updates customer roster and the daily schedule of services provided and allocation amount. Ensures compliance of meeting set outcomes and service targets set by supervisor.

Exhibits excellent customer service skills including returning customer calls in a timely manner and scheduling follow-up appointments when necessary.

Participates in Agency training as it relates to program operation.

Performs other essential duties and assignments as assigned by the Agency or the supervisor.

Qualifications:

Required: A Bachelor’s Degree from an accredited four-year college or university, good writing and documentation skills, and computer experience. Works well in a fast-paced environment and maintains confidentiality and objectivity. Demonstrates a high level of organization, planning, reporting skills, persistence, proactivity, listening, verbal communication, and building client relationships.

Pay: From $15.00 per hour
