The Buffalo Veterans Residential Program is to help US Veterans transition from homelessness to their own independent housing. Case managers work with the residents in assessing for areas of growth and skills to build on. Helping them to identify a person centered care plan that will ultimately help with their transition to permanent housing. Finally working with a group of residents weekly to help support them making progress on their goals. Veterans in the program encounter various barriers and case managers with basic understanding of mental health and substance use are encouraged to apply for this position.

Working in residential treatment is a team effort that benefits in being able to support the clinical team in working with the veterans in our program. Promoting a positive attitude helps in the development of the atmosphere that will help the residents recognize that there are opportunities to seize and transition towards independence they want. When working with residents in an individual setting helping them to identify the things they can accomplish themselves and to also be open to engaging in additional care, support, and services.

Requirements:

• Bachelor’s Degree preferably in human services
• Candidate will have a clean driving record
• Ability to utilize a computer using the Microsoft Office Suite
• Expected to maintain a client chart in a timely manner (e.g. document client progress in notes, assess progress and document, etc.)
• Engage with residents on daily basis through various interactions.
• Ensure veterans are following their plan of care and supporting them engaging with appropriate care.
• Administer drug and alcohol screens as the program does utilize observed urine screens
• Assist in helping when a veteran is in crisis
• 40 hours a week, with some weekend, evening, overtime, and holidays being worked
• Must pass pre-employment drug screen to be offered position
