Under supervision, provides overall support to DT program participants by teaching skills that foster independence and community integration as guided by the person’s individual service plan (ISP). Also ensures the highest quality of standards in a supportive, interactive, clean and safe environment.
Essential Duties and Responsibilities:
- Provides support to participants by teaching skills that foster independence and community integration.
- Follows the program plan as written in the ISP (i.e., goals, objectives, schedules, daily routines).
- Provides appropriate assistance to participants based on their skill level/need.
- Engages participants in activities.
- Completes and maintains accurate records of participants’ progress and services performed, reporting any changes in participants’ condition to Program Manager.
- Transports DT participants safely to locations outside of the programming area.
- Picks up and transports CILA participants safely from their residential areas to their day program sites and brings them home.
- Maintains vehicle in a safe and operable manner.
- Administer personal care, such as but not limited to ambulation, personal hygiene assistance, and client specific needs as delegated by the RNT or supervisor.
- Medication passing/monitoring and performs all medication passing/medical procedures as delegated by the RN or supervisor.
- Maintains a safe environment for participants.
- Adheres to St. Coletta organizational or departmental policies and procedures.
- Performs other duties as assigned
