Through hands on and supervisory work, the Community Site Leader ensures that the highest quality of person- centered care and support is provided to individuals living within the community home while expanding their independence. This position is responsible for providing an environment of encouragement, enhancement, and empowerment in areas including
- personal care
- physical environment
- community inclusion
- person centered care
- team supervision
- assessment/observation/reporting
- safety
Requires flexible schedule as shifts vary with some evenings and weekends. Must be 21 years of age or older with a valid driver's license. Currently DSP certified with at least 1 year experience, able to show proof of HS diploma/GED.
