V-Tech Solutions, Inc. is currently seeking qualified entry-level COMMUNITY HEALTH OUTREACH REPRESENTATIVES to assist us in providing targeted outreach to increase the rate of the Medicaid program's Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) encounters within the District of Columbia.

Duties and Responsibilities:
  • Responsible for face-to-face enrollee outreach per workflow
  • Accurate and timely documentation of outreach attempts, outcomes and status
  • Escalate barriers and/or concerns to Project Coordinator in a timely manner
  • Scheduling EPSDT visits for enrollees with appropriate or new PCP
  • Participation in medical and dental wellness days as assigned by Project Coordinator
  • Adherence to HIPAA verification requirements
  • Adhere to privacy requirements including secure protection of verbal and written personal health information (PHI)
  • Comprehension of the EPSDT program or the District's Health Check program and Periodicity Schedule
The successful candidate will need to be able to demonstrate the following knowledge/skills:

  • Excellent verbal and written communication skills
  • Cultural competence
  • Decision-making and problem-solving skills
  • Ability to take concise communicative notes
  • Ability to assess family strengths and identify needs for support
  • Ability to involve the family in health care decision-making
Minimum Requirements

  • Bachelors Degree in Public Health, Community Health, Social Sciences or similar field
  • Previous community health experience preferred
  • Must have driver's license and access to daily reliable transportation
  • Bilingual Spanish speaker is a plus
  • Familiarity with driving and parking in DC
