Hiring wage range: $57,100 – $60,526 annually
Full wage range: $57,100 - $68,520 annually

Job summary:

The Community Health Manager is responsible for supporting a team of 2-5 health navigators that provide comprehensive health navigation, including health insurance enrollment (public and marketplace), SNAP & WIC assistance, health insurance literacy, and connection to community resources. The Community Health Manager will audit all data entry and will provide training and support to ensure program outcomes are met. The Community Health Manager will build a strong team relationship, will maintain strong community partnerships, and will seek out new partnership opportunities.

Primary duties:

  • Increase access to health through equitable services and programming to improve positive health outcomes
  • Remain up to date on all healthcare systems, programs, bills and laws, changes, and policies
  • Capture and communicate community health trends in order to advocate for change
  • Participate on appropriate boards, forums, and meetings in order to raise awareness about programming and decrease barriers for clients
  • Manage budgets and ensure grant deliverables are being met, by auditing accuracy of data entry and by analyzing data on a regular basis and pulling and creating reports
  • Oversee Open Enrollment (OE) periods including scheduling and coordinating events, walk-in days, marketing, communication with front desk, training, and all other needs to ensure a seamless OE period
  • Manage all other staff appointments and time including medical assistances, case management and literacy

Responsibilities include but are not limited to:

  • Facilitate team meetings, one-on-one check ins, dial in days or walk-in events, and community events
  • Maintain up to date Stand Operating Procedures (SOPs) and provide on-going training and support for team
  • Represent FIRC at various community committees, taskforces and events while adhering to FIRC's mission, vision and values
  • Maintain strong partnerships with community organizations and statewide healthcare partners
  • Work in a team and communicate with honesty, integrity, and a sensitivity to the needs and differences of diverse cultures
  • Collaborate with program managers to enhance program delivery to best serve FIRC families and ensure highest quality of wrap around supports internally and externally are being met
  • Meet one-on-one with clients seeking support in community health navigation and SNAP as needed
  • Always maintain HIPAA and meet confidentiality standards
  • All other duties as assigned
Requirements:

General requirements of the position include:

  • GED or High School diploma required
  • Effectively demonstrate strong written and verbal communication skills in English required, and Spanish preferred
  • Bachelor’s degree in Social Work, Human Service, Public Health, or related field preferred; equivalent work experience in a related field will be accepted
  • Previous experience and knowledge navigating healthcare systems preferred
  • Two years previous supervisory experience preferred
  • Knowledge of community resources in order to make appropriate referrals
  • Time management and organizational skills to manage an independent schedule and competing priorities
  • Ability to utilize technology and databases to track program outcomes
  • Current Driver’s License and reliable year-round transportation is required
  • Pass a background check and e-verify
