Provide education on health topics to individuals and groups in a clinic or community setting to promote wellness and disease prevention under the direction of a registered dietitian (RD), registered nurse (RN) or other licensed provider.
  • Provide appropriate counseling to individuals, families and caregivers. Identify health education, health promotion and disease management needs.
  • Provide varied health education to individuals and groups in a clinic or community setting.
  • Will lead the organization and preparation of community events and presentations. Collaborate with Marketing and Provider Relations teams regarding community outreach events.
  • Promote/Teach basic disease states and health promotions for both patients and community.
  • Collaborate with other staff ( RD’s, NP’s, Cardiology) to problem solve and develop interventions. Maintain positive relationships with members, physicians and staff.
  • Organize supplies and resources for community outreach.
  • Support the implementation of educational and clinical programs. Promote programs to members, physicians and staff.
  • Ability to manage time, and resources. Work closely with Population Health & Prescription Management (PHARxM) and Clinical Services.
  • Support clinical performance and outcome programs.
  • Perform other duties as assigned.

  • Bachelor’s Degree in Health Science or related field, preferred. Certified Health Education Specialist (CHES) Certification and Certified Fitness Trainer (CFT) certification, preferred.
  • 1 year of clinical experience and 1 year of patient education experience, preferred.
  • Excellent verbal, written communication and interpersonal skills.
  • Knowledge of computer software (Microsoft Windows, Word and Excel).
  • Detail oriented. Ability to research and prepare teaching plan appropriate to subject matter and audience.
  • Able to function under pressure and perform multi-tasks.
  • Current California driver's license and proof of auto insurance.
  • Must be available to work evenings and Saturdays as appropriate to program needs.
  • BLS required within 60 days of hire.
  • Bilingual, Spanish preferred.
