The Role:
The Home Health Aide (HHA) provides personal care, comfort measures, and supportive care to patients in their homes as assigned by RN. The home health aide works under the direction and supervision of a registered nurse or therapist in accordance with an established plan of care. This position is not shift-based, but rather our Home Health Aides travel from patient to patient throughout the day.
The Responsibilities of the HHA:
· Provides care to the patient in accordance with the written assignment sheet.
· Assists the agency was compliance with all applicable federal, state and local regulations.
· Assists with and/or provides patient hygiene and comfort measures; bathing, positioning, oral hygiene, hair care, shaving, skincare, linen changes and nail care (on non-diabetic patients only) as directed by the registered nurse.
· Collects specimens as directed by the registered nurse.
· Obtains and records vital signs accurately and reports significant changes, as instructed, to the registered nurse promptly.
· Records intake and output and reports significant changes to the registered nurse promptly.
· Maintains proper body alignment, follows turning schedules for non-ambulatory patient and observes skin for signs of breakdown.
· Assists patient with ambulation, active/passive ROM and transfers under direction of registered nurse or therapist.
· Weighs patient and documents or reports weight as directed by registered nurse.
· Applies and removes anti-embolus stockings as directed.
· Assists the patient with toileting, colostomy care, catheter care, use of bed pans and urinals and maintains hygiene.
· Administer enemas as directed by the registered nurse.
· Performs simple procedures and medications normally self-administered, as assigned, and records results on the appropriate chart form.
· Recognizes and reports changes in patient’s condition and body functions to the registered nurse.
· Provides encouragement to the patient and promotes alertness and orientation through conversation and activity.
· Maintains confidentiality and all matters relating to the patient and agency.
· Recognizes an emergency situation and responds appropriately.
· Works cooperatively and communicates effectively with patient and family members and other members of the healthcare team.
· Recognizes physical, emotional, and developmental needs of patients and assist with meeting those needs appropriately.
· Documents patient visits appropriately and completes necessary documentation legibly and accurately in a timely matter.
· Understands and respects patient’s rights.
· Promptly reports and documents incidents, accidents, or safety concerns.
· Participates in patient care conferences and staff meetings, as assigned.
· Attends/participates in in-service programs and quality improvement activities, as assigned.
· Flexible to agency’s needs in regards to patient assessment and work schedule.
· Reports to work punctually and gives proper notice of absenteeism or tardiness according to agency policy.
· Uses appropriate infection control procedures and safety measures.
· Performs job functions and delivers services consistent with all applicable federal, state, and local regulations and the policies and procedures set forth in the agency compliance program.
· Performs other duties as assigned.
· Adheres to policies and procedures.
The Qualifications for the Position:
· Current Illinois Certified Nursing Assistant (CNA) licensure (required)
· One year of experience as a home health aide (preferred)
· Must have vehicle, current Illinois driver’s license, and automobile insurance in accordance with state regulations
· Successful completion of home health aide orientation program and competency evaluations
· Ability to read and complete necessary documentation appropriately, legibly and timely in accordance with regulations
· Ability to make home visits with the physical strength, range of motion, and dexterity to safely assist patients with activities of daily living, personal care, and transfers as required.
· Communication, interpersonal, and organizational skills necessary to complete job responsibilities
· An individual may not pose a “direct threat” to the health or safety of the individual or others in the workplace or in performance of job
· Ability to pass a criminal background check
