Position Summary:

The Clinical Nutrition Manager is responsible to identify and assess the nutritional needs of assigned patients/residents, initiate interventions to achieve and/or maintain individuals nutritional status, and enhance quality of life consistent with patient’s/resident’s goals. Will provide nutritional expertise to the health care team and educate patients/residents, staff or consultants. Will follow the Academy of Nutrition and Dietetics standards of practice and facility regulatory requirements.

Acute Care & Swing Beds

  • Provides diet consultations, nutritional assessments and nutrition analysis to residents/patients/families of all ages according to their ability to learn in accordance with facility policies.
  • Instructs and counsels residents/patients/family members on normal nutrition and therapeutic diets with consideration given to their education, readiness to learn, cultural and social-economic background. All education sessions will be documented in the medical record.
  • Develops and assists in presentations for group education classes at facility for residents/patients/families in relation to nutrition and therapeutic diets.
  • Assess patients for appropriate diet or nutrition support, documents and monitors these patients, making additional recommendations as needed.

All Areas of Practice

  • Interacts with other departments in setting up procedures, polices and problem solving as they relate to the patient/resident dining service and nutrition.
  • Actively participates in the development, review and revision of the continuous quality improvement programs and the facility wellness program.
  • Responsible for updating resident/patient diet, weights, food preferences and other patient data in the computerized tray card system (Surequest or equivalent).
  • Conducts regular and ongoing meal rounds, dining room assessments and test tray audits. Monitors and evaluates resident/patient satisfaction with food selection, preparation and service along with the Dining Services Director. Performs all other related duties as assigned by management.

Minimum Requirements:

  • Bachelors or Master’s degree from an accredited Nutrition or Dietetics program and Registration by the Academy of Nutrition and Dietetics.
  • Licensed/Certified per State requirements currently or upon hire.
  • Certification in approved sanitation course such as ServSafe upon hire.

Knowledge, Skills, and Abilities

  • Knowledge of The Joint Commission/ Critical Access/OBRA/other regulation specific to comprehensive assessments, care planning and residents rights.
  • Knowledgeable on the current medical nutritional therapy related to: diabetes, GI disorders, hypertension, renal disease, hepatic disease, nutrition support, cardiovascular disease, cerebral vascular disease and oncology.
  • Demonstrated ability to relate well to individuals and groups with widely varying backgrounds, perspectives, education, and skills is required. Ability to teach others is required.
  • Practice patient confidentiality, effective interpersonal skills, good judgment, and ability to listen with empathy and set priorities.
