The OASIS Program (Older Adult Services and Intervention Systems) has been helping seniors stay independent for over 30 years. We are advocates for the aging. We offer Case management services, information and referral as well as volunteer services to help seniors 60 + stay independent and in their own homes for as long as possible. The OASIS Program is seeking an individual that loves working with our aging population.
Responsibilities
- Provides case management and care planning to seniors 60+ in the Ventura, Oxnard and Port Hueneme communities.
- Coordinates delivery of services by making referrals including referrals for financial assistance to clients in need of emergency food, shelter, clothing, rental assistance, transportation, medical, and special needs.
- Reassess clients to determine if services are progressing according to plan.
- Serves as advocate with government agencies to obtain public assistance.
- Participates in outreach programs; represents Program/Region at community meetings.
- Helps prepare necessary reports, requisitions and statistics.
- Develops and implements as individualized care plan for each client.
- Provide case management follow-up and progress of clients to evaluate effectiveness of goals.
- Provides regular evaluation of client programs to ensure satisfactory progress.
Qualifications
- Bachelor’s degree preferred; completion of 2 years of college required plus one to three years of related experience.
- Excellent written, communication, public speaking, organizational and interpersonal skills.
- Able to work independently.
- Ability to work as a positive, enthusiastic member of a team.
- Proficient in Microsoft office.
- Spanish speaking a plus
Hours: P/T, 30 hours per week, (benefits offered) Monday-Thursday Must be able to travel within assigned communities.
Job Type: Part-time
