Provide case-management and assessment in both on-site and off-site settings; assures coordinated service delivery and successful client linkage with other programs and facilities; related duties as may be assigned.

Minimum Requirements: BA in mental health field and 1 year of post-degree relevant work experience. MSW w/LCSW preferred.

Special Requirements: Valid driver’s license in good standing. Bi-lingual (Spanish) helpful.
