Reports to: SSG Fox SPGP Program Manager
Coverage area: Throughout Chautauqua, Cattaraugus, & Allegany Counties - Will report to our main location in Buffalo one day per week, and a location in Olean, NY a half-day per week.
Responsibilities:
Case Managers are a key component of the SSG Fox Grant Program team responsible for engaging with Veterans, service members, and military families. This is achieved through community linkages with resources on a local, regional, and national level and coordination with SSG Fox Peer Support Specialists assigned to the same geographic area.
The Case Manager is responsible for maintaining a Veterans First mindset, maintaining accurate records and data integrity through a Salesforce-hosted database system, delivering quality and timely service, and continuously ensuring a high level of knowledge regarding potential resources. Specific responsibilities include, but are not limited to:
- Collaborate with peers and community partners to ensure seamless service to Veterans, service members, and military families.
- Maintain extensive awareness and knowledge of resources available for Veterans, service members, and military families as well as general community resources within their assigned geographic area.
- Perform participant needs assessments and screening for the SSG Fox Peer Support Grant Program and ensuring that all VA required eligibility screening is completed in accordance with regulations.
- Continuously screen for possible housing assistance and/or homelessness prevention, and ensure referral to appropriate resources within the local COC (Continuum of Care) including local SSVF and HUD-VASH providers.
- Creation of individual, integrated care plans.
- Developing client-guided, actionable, timely, and reasonable goals intended to address the participant’s obstacles to obtaining services.
- Conduct quality control to ensure quality service and data integrity.
- Continuously monitor personal workload and quality of notes.
- All other duties as assigned.
Qualifications/Knowledge/Skills:
- Enthusiastic about agency’s mission.
- Attention to detail is critical.
- Extensive Military/Veteran cultural competency.
- Ability to work with diverse populations.
- Proven ability to continuously deal with mentally and/or emotionally challenging situations.
- Experience in dealing with resources in the community.
- Extensive experience with modern office IT systems, including Microsoft Office suite - computer literacy is an absolute must.
- Salesforce or other database / CRM system familiarity a plus.
- Excellent communication (written and verbal), interpersonal, and organizational skills.
- The ability to multitask and prioritize workload in a fast paced environment is absolutely vital to this position.
- This position works closely with a team, and is expected to maintain constant communication with their team and supervisor to ensure that Veterans are being handled in the most professional manner possible, and in compliance with all SSG Fox Peer Support Grant Program regulations, VA regulations, and all State and Federal laws.
- Reliable transportation is a must – this position will cover a geographic area entailing three separate counties, and is expected to function primarily in a remote / mobile environment. Mileage reimbursement and all IT needs including phone, computer, and tablet are provided.
- Complete mandatory background check, training, and 90-day probationary period.
- All other duties as required.
