  • Works closely with program participants and case managers to assist clients. Provides direct services to clients, including intake, engagement, referral, advocacy, and follow-up, if appropriate.
  • Provides referrals for non-critical services including food pantries, local community resources, bus passes and access to telephone for client’s use for job search and medical appointments only.
  • Assists the case management team with caseload; monitor and document client progress towards goals as outlined in their individualized service plan.
  • Communicates any client specific special needs to the assigned case manager and provide accurate, complete, and timely data and reports to the Program Director.
  • Conducts and participates in regular case conferences or other meetings with clients and services providers as required.
  • Helps organize and lead new guest orientation programming.
  • Maintains thorough knowledge of current community and program resources and utilizes these to provide services to clients.

QUALIFICATIONS:

  • High school diploma required, associate’s degree preferred, plus 1+ year working with low-income population, or equivalent combination of education and experience.
  • Must have :
    • Intervention skills to resolve crisis and non-crisis situations in a timely manner.
    • Ability to learn the Social Service agency structure for assigned area.
    • Ability to assist in all aspects and operations of the YMCA Housing Program.
    • The ability to develop a rapport with individuals with special needs including people with mental illness, substance use and criminal histories.
    • Multi-tasking abilities.
  • Training requirements:
    • Immediately upon hire: American Red Cross First Aid, Oxygen Administration, AED and CPR-PR certifications
    • Within 1 year of hire (and every 2 years thereafter): Non-violent crisis intervention, community resources, cultural competency, child and elder abuse, ethics, mental health/substance abuse, motivational interviewing, and evacuation procedures.

Case Aide is responsible for providing support to the case management team and program
participants. Operates with a Housing First philosophy; utilizing Harm Reduction, Progressive
Engagement and Trauma Informed Care methodologies of client engagement and care recognizing
that all services are client driven. This person works directly with the program in providing access to
services, information and referral processes while assisting with crisis intervention. This position also
works closely with internal and external teams to meet the needs of program participants. This
position works closely with case managers and community service organizations in meeting the
housing needs of clients.
