Partners in Care is seeking a full time (field) Caregiver

The Caregiver works alongside the Home Care Aide, Registered Nurse, or other licensed staff members to provide safe and excellent patient care to hospice or home health patients in their place of residence. The caregiver assists with safe transfers, bathing, and other activities of daily living (ADL’s) in conjunction with licensed staff.

Partners In Care provides post-acute care to residents throughout a 10,000 square mile region of Central Oregon who are medically fragile, experiencing chronic and serious illness, recovering from surgery, or need end-of-life care at home or in our hospice in-patient facility called Hospice House. As an independent non-profit organization, Partners In Care works in close partnership with the patient or client’s family, the community, and other care organizations to ensure quality care.

Job Responsibilities

• Provides excellent customer service to all patients, families, caregivers, co-workers, and physicians.
• Provides patient care alongside licensed staff members in patient places of residence.
• Takes direction from licensed staff members when providing patient care.
• Learns and uses safe lifting and patient mobility techniques for two-person care of patients.
• Assists co-workers and others, as needed.
• Maintains strict confidentiality of patient and family information.
• Provides for a safe environment for patients, guests, visitors, and employees.
• Carries out duties and responsibilities of position in a manner that exemplifies excellent customer service.
