Full Job Description RN- Registered Nurse Adult

Salary: 95,000-105,000 Salary

Setting: Exciting opportunity to work with an amazing organization that provides community based health and wellness care to homeless New Yorkers. Join the Governor created Safe Options Support (SOS) team consisting of a Team Leader, Licensed Mental Health Clinicians, Care Managers, Peer Specialists, and a Registered Nurse.

Position: Very rewarding position working to help stabilize individuals in the community experiencing adverse events, including homelessness. Working within the SOS team, RN will address the physical/medical health needs of program participants in community settings and during their transition to permanent housing.

Daily life: Schedule is typically 9-5 Monday-Friday with some flexibility

Requirements:

• Graduate from an accredited school of nursing *BSN preferred
• Licensure and current registration as a Registered Professional Nurse in the State of New York required (RN)
• Registered Nurse with at least two years experience working with individuals with behavioral health needs.
• Comfort working with unhoused population in community setting

Responsibilities:

• Engagement begins either at known “hang-outs” or “Hot spots” within the transit system or during an inpatient hospital admission or emergency department visit with involvement in transition planning, including an assessment of community transition support essential to stabilizing the participant.
• Undertake, interpret, and monitor the assessment of participant’s physical, cognitive & psychosocial health and wellbeing needs.
• Plan, implement and evaluate care, interventions, and treatments in line with evidence-based practice to meet fundamental and complex care needs.
• Undertakes reassessment of participants when changing condition/needs/deterioration are identified
• Advise participants on health maintenance and disease prevention including effects of diet and physical activity and provide case management when indicated

Pay: $95,000.00 - $105,000.00 per year
