Associate Director Population Health Job at Tandigm Health LLC

Tandigm Health LLC Philadelphia, PA

Tandigm Health is a rapidly growing company transforming health care in the Greater Philadelphia area. Our mission is to help primary care physicians provide better care for their patients

SUMMARY

This position is accountable for maximizing the clinical, operating, and financial performance of Tandigm’s valued primary care physician network. The Associate Director (AD) will be assigned a geographic region within the five county Philadelphia area and hold accountability for the performance management, customer service, and training for all of the Tandigm Health private practices in that region. The AD will need to accommodate client availability for regular meetings with a flexible work schedule Monday through Friday and can expect to spend an average of 20-30 hours “on the road” each week visiting practices. Every candidate must have a valid U.S. State Driver’s License and personal vehicle for their own use.

The AD will guide physician practices in their transition from fee-for-service to value-based care as a population health subject matter expert, consulting on process changes that positively disrupt the status quo and allow practices to improve the patient experience, realize improved financial rewards, and contribute to a net positive impact on the care delivered in the greater Philadelphia area. This role will provide expert analysis of Tandigm Health data to help practices “connect the dots” linking process improvement with improved patient healthcare outcomes.

The ideal candidate, while comfortable working unsupervised, will demonstrate a strong sense of collaboration with clients and teammates, an ability to foster positive relationships, and excellent leadership capabilities. Candidates must also have a high level of emotional intelligence and demonstrated facility in change management as well as in navigating client “office politics.” A passion for being “part of the solution” in helping improve healthcare is an absolute must.
