Community Mediation Services is looking for a Full-time Case for our Brief Strategic Family Therapy (BSFT) program, who will be helping to support BSFT staff who are responsible for providing counseling, mentoring and service plans for families and youth between ages 0 – 18 in Queens and Brooklyn.

Job Description:

  • Support BSFT families and adolescents who are at risk or, experimenting or using substance.
  • Attend home visits / community visits with client and assess safety.
  • Help client navigate systems (ie. HRA, housing).
  • Act as an advocate for clients while on community visits.
  • Escort clients to appointments when needed.
  • Collaborate with Therapist on case needs and assist with case management.
  • Participate in case conferences.
  • Conduct a combination of home and office visits, tele-conference and when necessary phone sessions
  • Call agencies and collect information regarding referrals made and appointments kept.
  • Provide support to BSFT Therapist on cases in Brooklyn and Queens

Comply with all record keeping & documentation requirements

  • Document timely and accurately progress notes and client records obtained.
  • Complete all related documentation included in Case Aide folder.
  • Maintain case records and record keeping in compliance with ACS requirements/and all existing Federal, State and City laws. This includes CONNECTIONS and PROMIS systems.
  • Request school and medical records.

· Assist in preparing cases for audits and case closure.
Qualifications and position requirements

  • Associate Degree required/ Bachelor Degree preferred; Bilingual Spanish preferred.
  • Up to 1 year experience working with families or in a social serve field, preferred.
  • Strong interpersonal and writing skills.
  • Data entry experience, knowledge of Microsoft Word.
  • Ability to handle various projects at one time.
  • Willing to work in the homes and community
  • Be available for client emergencies and crises during evenings/weekends.
  • Optimistic about change and have a “do what it takes” attitude.
  • Must be able to work effectively with a team.
  • Work schedule based on program needs and is subject to change where necessary.

Salary range 34,000 to 36,000

  • MUST SHOW PROOF OF COVID VACCINE

Pay: $34,000.00 - $36,000.00 per year
