Citizens Care & Rehabilitation Center is where Care Begins with Compassion. We are seeking a Full Time Activities Assistant on our Day Shift (9:00AM to 5:30PM), 40 hours/week with an every other weekend requirement.

The Activities Assistant is responsible for organizing and executing the resident and guest recreational programs in coordination with the Director to include, but not limited to physical, social, religious, games, intellectual and creative arts and crafts.

DEMENTIA EXPERIENCE IS A PLUS!

Essential Responsibilities/Duties:

· Leads group programs as directed by the Director of Activities.

· Assists in planning individual community calendars.

· Completes the proper assessments and forms for each resident or guest in the assigned community.

· Writes an individualized Care Plan for each resident and guest in the community.

· Follows the Care Plan to implement the resident’s and guest’s social and recreational programs.

· Reports to the Director of Activities responses made by residents or guests and/or progress to specific approaches followed. Maintains written records.

· Maintains an on-going inventory of departmental supplies and equipment maintenance. Reports needs to the Director of Activities timely.

· Completes requisitions for supplies.

· Performs duties as a Universal Worker to include responsibilities such as general housekeeping duties in the common areas and returning equipment and/or supplies to the appropriate storage areas.

· Assists residents and guests by transporting them to and from programs.

· Communicates with nursing staff the residents and guests participating in functions for the purpose of assistance in transporting.

· Recognizes abnormal signs and symptoms which signify a physical change in the resident’s condition and which require a nurse’s care.

· Alert for conditions that may interfere with the safety and well-being of the residents and guests and reports hazardous conditions to the Director of Activities and/or the RCC immediately.

· Assists with tray passes in the community.

· Assists with feeding residents after participating in appropriate education.

· Attends In-Service training to maintain and improve competencies.

· Other duties as assigned.

Education/Experience:

High School Graduate or GED equivalent is required. One year experience in a recreation setting is preferred.

Knowledge/Skills/Abilities:

· Promotes positive image of our facility to residents, guests, families and the community.

· Protects the privacy of residents, guests and employees.

· Must have a basic understanding and empathetic approach to the needs of the ill elderly.

· Must have willingness, patience and an aptitude for working with people.

· Must exhibit the ability to give instruction to others and to lead in a calm and unhurried manner.

· Exhibits imagination, creativity and “thinking outside the box”.

· Exhibits dependability and punctuality when working as scheduled.

· Requires excellent verbal and written communications skills, and a professional demeanor.

· Projects good judgment and performs all duties satisfactorily while prioritizing projects as assigned.

· Exhibits a spirit of cooperation and has a pleasant, friendly personality.

· Exhibits excellent customer service skills and attention to detail.

Physical and Environmental Requirements:

Must be able to stand, sit, stoop, push, pull, balance, reach, grasp, kneel, climb and lift up to 50 pounds. Exposure to soft coated wheaten terrier (facility dog).

The Activities Assistant must meet the physical requirements of the position which include transporting residents in wheelchairs to/from programs as well as on long walks. The Activities Assistant also runs light fitness programs while seated.
