Job Type: Part-time

Schedule: Weekends 10-6

Pay: $19-25 Per Hour

We are leveraging the latest technology to bring advanced diagnostics to our patients, at the Point-of-Care, seamlessly integrated with our Electronic Health Record (EHR). We combine the strength of our team, technology, and operational support to streamline the patient experience, eliminating the delays, barriers, and frustrations all too common in healthcare.

We currently have multiple Care Captain positions available.

Responsibilities:

  • Forms a personal relationship with patients, and periodically check in with patients
  • Communicates with patients before and after visits
  • Answers patients’ general questions and facilitate communication with providers
  • Assists patients with reviewing results, scheduling follow ups, and requesting med refills, etc
  • Helps patients with referrals and coordinate internal appointments
  • Coaches patients to achieve the health goals they establish with providers
  • Helps patients with insurance questions and issues
  • Reviews open patient cases and Athena inboxes for the practice, ensuring timely responses
  • Escalate any immediate health concerns/issues that may arise to provider
  • Conduct morning huddles with staff and provider to communicate any changes/updates
  • Securely document, maintain, and update electronic patient records using assigned devices.
  • Ensure smooth, efficient clinic services and maximize comfort for the patient
  • Maintain privacy and confidentiality of patient records, according to HIPAA and compliance standards
  • Identify and engage the community through schools, social services, and faith-based organizations
  • Plan and coordinate outreach activities in designated areas that are intended to identify, and capture the attention and interest of the families we serve
  • Attend the following events: Health Fairs/Community Events, Community Fairs, Door to Door Engagement, Flyer Distribution, Specific Site Engagement, and Recruitment.
  • Maintain outreach logs and complete tracking tools in a timely manner Perform other related duties as directed or required.

Requirements:

  • High school diploma or equivalent
  • A bachelor’s degree in a healthcare-related field is preferred but not required
  • Knowledge of the inner workings of the healthcare system

Work Environment

Well-lighted medical office or reception area. Exposure to communicable diseases and other conditions related to clinic setting. Work may be stressful due to a busy office.

Mental/Physical Requirements

Must possess the physical and mental abilities to perform the tasks normally associated with a receptionist involving sitting approximately 90% of the day with occasional standing, walking, reaching, and lifting. Periodic stress occurs from handling many calls and dealing with patient requests.”
