- *Starting wages are higher depending on experience and position (RN/LPN)::
- Hold a current LPN or RN license in good standing with the Board of Nursing (Required)
- Experience with maintaining and carrying out the Infection Control and Antibiotic Stewardship Program (preferred)
- Experience with Matrix Care (preferred)
- Skilled Nursing Experience: 3 years (preferred)
- Assisting DON with managing the staffing schedule for the unit, including assigning tasks and duties to nursing staff
- Assisting DON with ensuring Bermuda Village Health Center is in compliance with relevant regulations, laws and accreditation standards
- Coordinating care for residents, including coordinating with doctors, therapists, and other healthcare professionals to ensure that residents receive the necessary treatments and therapies
- Communicating with residents and their families to keep them informed about their care and progress
- Participating in the development and implementation of care plans for residents
- Performing medical record audits/reports to maintain accurate and complete medical records for residents
- Ensuring nursing staff admission and discharge processes are complete and met within 48 hours
- Organize staff education, training and support as needed
- Assist in insuring units are well-maintained and that all necessary supplies and equipment are available
- Audit missed medications/treatments
- Audit V/S compliance
- Audit CNA documentation compliance
- Audit Nursing documentation compliance
- Tracking patient falls
- TB documentation and screening
- Narcotic tracking
- Review all new physician orders
- Inspection of medication cart and follow up with pharmacist monthly inspection findings
- Audit patient charts to ensure HP, D/C summary and all necessary assessments and forms are in compliance
- Prepare statistical data for QAPI (quality assurance process improvement) meetings (falls, , infections, reportable wounds, other key performance indicators as identified).
- Responsible for ensuring that all employees are providing excellent customer service to internal and external customers.
- Perform job duties for residents and with team members in a courteous and professional manner.
- Take initiative to ensure resident safety and satisfaction is a priority.
- Meet and greet visitors in a friendly, helpful manner (Visitors include anyone who visits the community; specifically, current residents’ families and friends, prospective residents and their families, referral sources, vendors and regulators).
- Answer phones appropriately, according to company and community standards.:
- Ensure compliance and understanding of all regulations regarding residents’ rights.
- HIPPA guidelines and regulations:
- Follows communicates company policies and procedures
- The job description provides a framework for the job; other duties may be assigned as necessary.
- May work in other positions temporarily when necessary.
- Is subject to callback during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
- Is subject to injury from falls, burns from equipment, odors, etc., through the workday, as well as to reactions from dust, disinfectants, and other air contaminants.
- May be subject to the handling of and exposure to hazardous chemicals.:
- Medium work: ability to exert 10-35 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to lift, carry, push, pull, or otherwise move objects.
- Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents and staff.
- Must be able to relate to and work with the ill, disabled, elderly, emotionally upset and, at times, hostile people within the community.
- Clean, well lit, comfortable environment (personal office space).
- Exposure to sights and smells associated with medical treatments in progress.
- Low to Moderate noise environment.
