Thursday - Sunday - Outpatient Program Manager Job at Helping Hearts Residential Facilities LLC
Who We Are:
Helping Hearts provides behavioral health services in single family residences. We have been proudly serving the mental health community since 2008. We have many shifts available at our multiple locations in the Phoenix area. We are looking for compassionate staff to join our team. We prefer experience; however, we recognize traditional work experience is not the only form of experience. All staff will complete a new hire training that will provide for Arizona State Caregiving Certification, CPR/First Aid and CPI Certification.
Essential Duties and Responsibilities:
Dutiesinclude, but are not limited to the following:
- Provide coordination of care with behavioral health and medical providers which can include transportation to physician appointments, Urgent Care, Emergency Room and hospital settings and communicate all information and follow up needs to the Clinical Liaison.
- Organize, provide transportation, support and oversight for the clients and facilitate planned activities at the Clubhouse and in the community and day program groups.
- Provide transportation, support and oversight for the clients as they integrate into the community. This includes community functions such as shopping and banking, opportunities for moderate exercise such as walks, hikes and swimming and engagement at outpatient clinics.
- Provide personal care services to clients, following the authorized client service plan.
- Provide support and oversight of clients in alternate setting, such as short-term hospitalization.
- Provide support to the Clinical Liaisons and Direct Care Staff, as needed, to maximize coordination of care and create effective treatment plan implementation.
- Provide therapeutic support to the clients to develop independent living skills such as symptom management, budgeting, meal preparation and life skills.
- Provide assistance with the medication process including the delivery of prescriptions and the pickup and delivery of medication from the pharmacy.
- Assist with preparing and fulfilling shopping orders and supplies lists, facilitate vehicle maintenance, distribution of documents and program forms, etc.
- Perform any other duties, as deemed necessary and as directed by a member of the management team, including shift fills as needed.
**Salary starts at $17.00 per hour depending on experience.
**Weekend shift: Thursday - Sunday 7a - 7pm

Who We Need:
Compassionate staff who are dependable, patient reliable, adaptable, detail oriented and can multi-task. At times, staff will need to assist in de-escalation of behaviors (i.e. physical aggression, verbal aggression) and other crisis situations and be able to accurately document in the electronic health records what they observe with their member interactions.
We are looking for applicants who are:
- At Least 18 years of age
- Have a minimum of high school diploma or GED (must provide copy of document)
- Have a clean driving record within the last 39 months
- Are fluent in speaking and writing in English
- Possess an Arizona Level 1 Fingerprint Clearance Card (must provide before hire)
- Proof of ability to work in the United States
- Ability to pass drug screening and physical
- Have reliable transportation to and from work
- Can type 25 words per minute and have basic computer skills
- Must have sufficient physical strength and mobility to assist a participant to transfer from a wheelchair to another chair or commode without assistance and assist with a two-person transfer.
- Personal qualities including flexibility, tolerance of varied lifestyles and behavior, sound judgment.
- Prefer at least 2 years’ experience in related field such as: Behavioral Health, Mental Health, BHT (Behavioral Health Technician), Caregiver, CNA (Certified Nursing Assistant), Certified Caregiver, PCS, or group home experience
