Position reports to: Executive Director
Job Requirements:
¨ Minimum of two (2) years’ experience with the target population and knowledge and understanding of available services of those with developmental disabilities
¨ Ability to communicate effectively in both written form and verbally
¨ Ability to work independently with minimal supervision
¨ Criminal background check with no history of disqualifying offenses
Essential Job Functions:
· Flexibility in work hours and the ability to work independently
· Due to the necessity of the support broker to work face-to-face with consumers, families and support coordinators and to conduct trainings regular and predictable attendance is an essential job function
· Management of Self-Directed Services in conjunction with the consumer and/or guardian.
· Recruit, hire, counseling and management of staff
· Understanding of the roles of the DR and assisting him/her in the fulfillment of those duties
· Assist designated representative (DR) with timesheets and approval, conflict resolution, problem solving, navigating the computer programs of the SDS provider, paperwork and understanding documentation requirements, management of budget and staff rate setting, development of emergency back-up plan,
· Organize and maintain documents
· Establish work schedules for staff
· Knowledge of the supports and resources for those with developmental disabilities
· Facilitation of meetings with families and consumers
· Measurement and reporting of consumer progress on their outcomes.
· Thorough knowledge of the consumers’ Individual Service Plan (ISP) and the ability to train staff to be versed in the contents
· Timely documentation of all services rendered
Essential Job Duties:
· Provide needed services to consumers in a timely manner
· Document services provided in the Electronic Medical Record (Therap) no later than 5 days after the end of each pay period for all hours worked during the previous pay period.
· Any non-billable hours worked should also be entered into Therap and noted as non-billable.
· Therap will be used to calculate payroll. Hours paid will be solely based on documentation submitted. These entries will constitute the employee’s timesheet. No payroll will be paid unless documentation is submitted.
Job Types: Full-time, Part-time
Pay: $20.00 - $22.00 per hour
