Speech Language Pathologist Job at Attendant Care Companies
The Speech Therapist plans and provides speech therapy services to clients in their homes in accordance with Plan of Care and Agency policies and procedures. The Speech Therapist performs functions that require substantial specialized knowledge, judgment, and skill and must be able to make judgments accordingly.
Roles and Responsibilities
As a Speech Therapist, some of the duties will include:
- Implements physician-ordered rehabilitative therapies for speech function, auditory and visual comprehension, reading and writing ability, and swallowing function including:
- Evaluating and diagnosing speech, language, communication, and swallowing disorders
- Treating speech, language, communication, and swallowing disorders
- Providing training and education to family/caregivers and other professionals
- Evaluating results of speech therapy by observing, noting, and evaluating patient’s progress; recommending and implementing adjustments and modifications
- Completing discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contributing to patient care conferences
- Assuring continuation of therapeutic plan following discharge by designing and instructing equipment; recommending outpatient or home health follow-up programs
- Documenting patient care services by charting in patient and department records * Performs speech therapy evaluations, diagnostic tests, skilled procedures, and ongoing evaluation for clients who are receiving services under a medically approved Plan of Care.
- Completes comprehensive evaluation within forty-eight (48) hours of physician referral for speech therapy services unless other arrangements are made.
- Develops a speech therapy care plan; reviews and updates per Agency policy and client need.
- Documents all findings, plans, interventions, and outcomes and submits documentation to the Agency within seven (7) days or per Agency policy.
- Communicates plans and changes to the physician and to the client Case Manager and other caregivers through the care plan, progress notes, and participation in care conferences.
- Communicates changes in schedule and physician orders to the office on the day the changes are made.
- Plans, teaches, supervises, and counsels clients about their speech therapy needs to promote improvement or recovery, symptom management of disease process, and community resources.
- Evaluates the effectiveness of the teaching plan and establishes care plan for others to follow in teaching the client.
- Participates in discharge planning and documents this in client record.
- Performs other related duties and responsibilities as deemed necessary.
Requirements
The Speech Therapist will need to have the following items:
- Valid State of Michigan’s Driver’s License
- Current Auto Insurance
- Current CPR certification.
- Current Speech Therapy Registration
- Graduate of an accredited Speech Therapy program.
- State-licensed ST with 1-year experience, within the last 5 years, of Home Health Care
