Responsibilities

• Serves as the lead and clinical resource for all social workers.
• Collaborates with other departments in order to improve social work services for patients and families.
• Manages oversight of Palliative Care Coordinator role and works with Director of Palliative Care to coordinate expansion of palliative services, contracts, and programs.
• Licensed Clinical Social Worker performs assessments and interventions with palliative patients and families within scope of practice as a member of the interdisciplinary team.
• Performs duties that include assessment, intervention, care planning, outcome evaluation, documentation and collaboration/communication with the Interdisciplinary Team to assist patients and families with their goals, and assist the team in the delivery of Palliative care to the patient and family.
• May supervise interns and unlicensed social workers.

Qualifications

• Education/Regulatory Requirements:
• Graduate with a Master’s degree from an approved college or university accredited by the Council on Social Work Education.
• Must hold a current license in the State of Florida as a Licensed Clinical Social Worker.

Skills:

• Minimum of 5 years Palliative, Hospice or Medical Social Work experience preferred.
• Expertise in social services, community resources, crisis intervention, and family dynamics in a health care setting.
• Exhibits characteristics of leadership, team building, self-motivation, creativity, flexibility, and nurturing, while also possessing good communication skills in a variety of professional, emotional, and clinical settings.
• Works effectively and with flexibility as part of a team

Professional Requirements:

• Skilled in written and oral communications..

Language Skills:

• Bilingual (English/Spanish) preferred but not required.

Job Type: Full-time

Pay: $55,000.00 - $70,000.00 per year
