SUMMARY: The West Hollywood Multi-Disciplinary Team (WeHo-MIT), OCS, FSP, programs provide intensive mental health services to Adults (18-59 years) in community, field-based settings. Members are generally homeless or at risk of homelessnessexperiencing severe mental illness, and co-occurring substance disordersat enrollment or are discharging from a hospital or jail mental health setting prior to enrollment. The team provide services to all members on the census with “whatever it takes and is appropriate” including adequate outreach and engagement services prior to enrollment to ensure services will be successful. Step Up as an agency subscribes to a Trauma Informed Model of Rehabilitation, Housing First, and Wellness and Recovery for all members.

The is a multi-disciplinary team inclusive of a Program Manager, Clinicians, Housing Coordinator, Substance Use Counselors, and Peer Support that provides field-based services in the community. The Service Coordinator I provides both agency and field-based rehabilitation, linkage, and case management services to support members in obtaining and maintaining housing. The WeHo-MIT team works toward assisting those members in improving or maintaining their current level of functioning in the community.

HOURS: FULL-TIME

CLASSIFICATION: NON-EXEMPT (HOURLY)

DUTIES: The following reflects essential functions for this job but does not restrict other tasks, which may be assigned:

Homeless Support Services

· Support the targeted outreach efforts of the WeHo-MIT to initiate contact, develop rapport, and in coordination with Multidisciplinary team members: SC II, SC I, Substance Abuse Specialist, and PM, will assess homeless adults for mental illness, co-occurring substance use and other health care needs in the community until linkage and services are accepted.

· Complete the homeless survey tool, The Packet (formerly known as the VI-SPDAT) on all homeless individuals encountered to determine severity of need. Submit tool results to local CES for entry and housing match.

Leadership, Administration, Oversight

· Attend, participate, and collaborate in weekly team meetings

· Attend and participate in administrative and supportive supervisions

· Attend trainings relevant to the work and population to expand professional development

Direct Service and Support

  • Outreach and engage to potential members to engage and help evaluate for appropriateness and desire for services in jails, hospitals or street settings.
  • Engage currently enrolled members in treatment plan activities and support them in their recovery as they move towards a higher level of wellness. Coordinate with SCII, the appropriate level of care and utilization of resources

· Provide all supportive services, mental health rehabilitation and case management services, within scope of practice, to support member’s recovery.

  • Ensure each member has applied for or is receiving mainstream benefits. Assist with benefits establishment process, including health care enrollment and other needs such as housing applications, employment or vocational training, continuing education etc.
  • Ensure access to local resources, including psychiatric and medical care, housing, rehabilitation programs, drug/alcohol services, socialization activities, providing transportation and accompanying the client when necessary.
  • Communicate regularly with other treatment providers (internal & external)
  • Conduct ongoing needs assessments and support client in accessing and utilizing supportive services to meet their needs including access to housing, food, transportation, discount utility services, clothing, etc.
  • Make home visits on a consistent basis to support independent living skills and subsidized housing certificates. Assist with housing inspection readiness, Housing Authority issues, rent qualifications documentation, etc.

· Use trauma informed, motivational interviewing, housing first and recovery-oriented models of treatment.

· Conduct risk assessments for individuals in crisis and participate in After Hours Crisis call rotation

Documentation and Data Collection

· Complete all DMH chart paperwork and maintain documentation according to Medi-Cal standards of practice in Electronic Health Record – Welligent within specific timeframes as outlined in SUOS documentation policy.

· Support team in completing Outcome Measure Application forms to track member progress and stay in compliance with DMH charting standards

· Meet Monthly and Quarterly billing expectations

Job Type: Full-time

Pay: $21.15 - $25.00 per hour
