Service Coordinator Job at Greater Nashua Mental Health
Greater Nashua Mental Health is looking for a full-time Coordinator to join our Substance Use Disorder Services Team! The Coordinator oversees the case management and peer recovery support services components of the program, providing administrative supervision for the Case Management and Peer Recovery Support Staff. Manages special projects and initiatives as assigned to meet contractual requirements and improve program service delivery. Acts as a liaison with other providers to decrease barriers to care, promote ongoing care coordination, and ensure census development. Ensures all reporting requirements for DHHS and BDAS are completed as indicated.
Duties & Responsibilities:
- Develops and oversees Case Management and Peer Recovery Support programming to ensure high-quality services and fidelity to evidence-based practices, including provision of administrative supervision to Case Manager(s) and Peer Recovery Specialist(s).
- Manages the referral and intake process to ensure timely access to care.
- Ensures a high-quality outpatient program, by supporting program compliance with federal and state regulations and contractual requirements through chart auditing, participation in Quality Assurance projects, and staff training.
- Manages the drug testing program and various other projects as assigned.
- Ensures grant requirements are met by overseeing data collection efforts and preparing required reports within specified timeframes.
- Coordinates with GNMH staff in other departments to develop and implement effective workflows to decrease barriers to SUD treatment for GNMH clients and to enhance integration of care.
- Serves as a liaison with community partners to decrease barriers to SUD treatment and to enhance client access to recovery-oriented supports and services.
- Provides outreach and education to community partners to assist with census growth.
- Ensures appropriate staffing for Case Management and Peer Recovery Support Services including hiring staff, maintaining accurate job descriptions, and completing reviews of staff performance consistent with the department's and GNMH's goals and procedures.
- Complies with all facility, personnel, safety, infection control, and security policies and procedures.
- Performs other duties or special projects as required or assigned.
Qualifications:
- Bachelor’s Degree in Social Work, Human Services, Psychology or other related field. Master’s degree preferred.
- 3-5 years of experience working with people diagnosed with mental health disorders, co-occurring severe mental illness, and substance use disorders.
- Ability to lead and guide staff. Flexible, willing to learn, and anticipates the needs of self and staff.
- Familiarity with community resources.
- Ability to coach / teach effectively.
- Demonstrated skills in crisis intervention.
- Strong collaborating skills, and an ability to work effectively both independently and as part of a team.
- Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
- Excellent interpersonal skills and verbal/written communication.
- Ability to multitask, and be flexible with regard to workload and assignments.
- Ability to problem solve and exercise judgment.
- Excellent organizational skills
- Knowledge of Microsoft Office products (Email, Word, EXCEL).
