1 Full Time position for Monday – Friday 8:30am to 5:00pm in Round Mountain

1 Full Time float position for Monday – Friday 12:00pm-9:00pm FLOAT

1 Part Time position for Saturday & Sunday 11:00am-9:00pm in Redding


POSITION SUMMARY:

Safety and Support Team Navigators (SSTN) provide engagement and support services, particularly to program participants engaging in behaviors that pose health and safety risks, by meeting individuals “where they are at” and providing non-judgmental, non-coercive services, and resources to individuals. The Safety and Support Team Navigator is a leader in promoting a commitment to providing services using a harm reduction approach.


SSTNs work primarily in clinic lobby’s, and can be called to any part of the building, to create and maintain safe and supportive environments where both staff and participants have meaningful positive experiences. Our services are often the last open place for people to access care and develop therapeutic relationships with providers. Our approach is to provide kindness in all interactions with all people.


At times an escalated person will require specific attention to de-escalate and staff will need extra support to accomplish. SSTNs are that extra support. Using training provided to you by HCCC and your own compassion, your primary role will be to apply those to each interaction with an aim to reduce potential for harm.

This employee will work 40 hours per week as assigned, to staff one of our four clinic locations. The locations are: 29632 Hwy 299 E, Round Mountain, CA 96084, 1201 Industrial St. Redding, CA 96002, and 1401 Gold St. Redding, CA 96001.

The job requires direct advocacy work with participants who may use drugs, have mental illness or other chronic difficulties. The job requires a background check for criminal arrests and convictions. A criminal record does not necessarily disqualify an applicant, each case will be considered on its own merits. A safe driving record is a must. This employee must have a valid CA driver’s license and insurance.

The job responsibilities include lifting, cleaning, walking up to a mile several times a day (rain or shine), and climbing stairs. This staff person will drive a company vehicle to and from sites around the area as necessary.

Duties will include risk assessment and risk reduction, de-escalation, safety, support, participant advocacy and navigation, and referrals to other services in the community.

BASIC REQUIREMENTS:


  • Minimum 2 years’ experience working within a participant centered environment, effectively de-escalating elevated people and situations. (personal lived experience may substitute professional work)
  • Physical health that allows the employee to use public and private transportation, to walk distances up to 1 mile several times a day (rain or shine) and work their assigned tasks for periods up to 8 hours per day with appropriate breaks. Duties may occasionally include lifting up to 20 pounds, climbing a flight of stairs and light cleaning of the office area.
  • Literacy that allows the worker to complete record keeping forms necessary to the data collection and reporting associated with this project. This may involve report writing and entering information into Electronic Health Record system.
  • Ability to develop relationships with participants and other providers is key to the program’s success.
  • Effective problem solving skills and experience dealing with diverse individuals and communities. They will have the skills necessary to display person-centered, non-judgmental services to all prospective participants, and staff.
  • As a SSTN, you need excellent verbal communication skills to discuss building security with clients and de-escalate situations. It may also be useful when making announcements to visitors, exchanging information with colleagues, and recalling details of events.
  • Strong attention to detail: working as an SSTN requires this skill to be alert to safety events.
  • Teamwork is essential to the work we do. One must collaborate with others to reach shared positive outcomes.
  • Licensed California driver with good standing driving record

  • Experience and knowledge involving community services, case management, medical transport, eligibility work, referral development and participant advocacy.
  • Effective communication ability in Spanish.
  • Experience working in harm reduction programs or syringe service programs.
  • Experience in maintaining safety and security
  • First Aid/CPR Certified
  • Certification in: De-Escalation, MHFA, Motivational Interviewing
  • Experience in case management, patient navigation, advocacy


REQUIRED TASKS:

The worker will perform all of the following tasks:

  • Report to work on time and complete all assigned hours and tasks. Assigned hours may include weekend and evenings. If the worker is not able to report to work for any reason they are to contact their supervisor by phone in advance to say that they are not available for work. Poor attendance or the lack of achievement of assigned objectives will be cause for employee counseling sessions and other personnel action, including termination, as necessary.
  • Coordinate with staff and vehicles used in the community and outreach programs.
  • Be responsible for assigned record keeping and data reporting for their project responsibilities.
  • Work with all participants following the organizations guidelines, which require non-judgmental, caring interactions. All HCCC staff, interns and volunteers who are associated with the organization will refrain from inappropriate interactions with participants, including buying or selling items, lending money or participating in personal relationships outside of the workplace or business hours.
  • Provide each individual that you encounter with compassion, dignity, and respect.
  • Collect and enter data to be used in the evaluation of program effectiveness and work with others in creating evaluation reports for use in assessing project accomplishments.
  • Collaborate with all staff in providing connected and effective services that support efforts in providing harm reduction based quality care.
  • Attend all assigned staff and team meetings, and participate appropriately in the planning and review of project activities.
  • Participate in on-going tasks involved with data collection and participate in working groups in support of the quality management process. Complete other appropriate tasks as assigned by supervisors in carrying out the project.

The work environment is a, mostly indoor, clinic and business office. The noise level in the work environment is usually moderate with normal business office noise, as well as patient interaction and waiting room noise levels.


The lobby serves as the waiting area for all patients and participants of programs at HCCC.
