RN Team Supervisor – ACO Value-Based Care

ACP is seeking candidates to join our Abacus Accountable Care Organization (ACO) as the RN Supervisor for our Care Coordination Team. We have a team of care coordinators who provide exceptional services, and we need a hands-on supervisor with great people-aptitude and clinical patient experience to help lead them. This position will manage a team of care coordinators with responsibilities for providing clinical direction, hands-on guidance, and supervision support.

Work hours are Monday – Friday and our work environment is professional with casual dress (we love our denim jeans).

This position will work under the direction of the ACO Director and ACO Medical Director in the areas of Care Management and Population Health.

Responsibilities

· Manages staff, providing clinical direction and general supervision of team including hiring and training of new staff, mentoring, evaluating performance, addressing performance or team concerns, delegating work assignments effectively

· Provides leadership in resolving concerns about service delivery, patient, and provider needs

· Coordinates program service delivery

· Contributes to the design of quality outcome measures and manages tracking and reporting needs

Qualifications

Required

· Current Register Nurse (RN) license permitting work in the State of Arizona

· Supervision experience preferably in healthcare, including training staff

· Excellent presentation skills with the ability to manage questions and to work with a diverse employee population

· Proficient use of Microsoft Office and experience with Electronic Health Records (EHR) systems, preferably EPIC

· Ability to respond to sensitive and/or confidential inquiries or complaints from regulatory agencies, or members of the business community

· Effective written and verbal communication skills including professionalism and courtesy

Preferred

· Knowledge of care management techniques, tools and responses required to ensure optimal patient care

· Knowledge of processes and procedures for patient and population management

· Skill in evaluating program and individual performance and recommending improvements

Job Type: Full-time

Pay: $1.00 - $2.00 per hour

Schedule:

• 8 hour shift
• Monday to Friday

Experience:

• Nursing: 1 year (Preferred)

License/Certification:

• RN (Preferred)
