RN Case Manager Hospice Job at HIGH PLAINS SENIOR CARE
High Plains Senior Care Hospice is looking to hire a compassionate RN, with superior leadership and time management skills. Should possess the ability to maintain strong, professional relationships in the community, as well as with our care team. The Case Manager is responsible for managing and providing high levels of coordination between patients, families, and the interdisciplinary team, as ordered by the physician. This is a flexible position, with a supportive team, that aides in providing our patients and their families with compassionate care.

Qualifications:
Current RN Licensure in the state of Texas, or Compact License
Preferred but not required, one year experience of nursing care in a hospital, home health or hospice
setting. Willing to provide on the job training.
Current driver's license, vehicle insurance, and access to a dependable transportation.
Environment:
Works in patient's homes, nursing homes, assisted living facilities, hospitals, clinical offices and other
environments in various conditions; ability to travel locally and perform on call duties.
Job Type: Full-time
Pay: $70,000.00 - $75,000.00 per year
